Ketoconazole
Indications/Uses
Systemic fungal infections eg, blastomycosis, chromomycosis, coccidioidomycosis, paracoccidioidomycosis; in patients who have failed or intolerant to other therapies.
Dosage/Direction for Use
Min duration of treatment: Candidiasis 1-2 wk, recalcitrant dermatophyte infections 4 wk, systemic mycoses 6 mth. Adult Initially 200 mg once daily. May be increased to 400 mg once daily. Childn ≥2 yr 3.3-6.6 mg/kg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Not to be used for CNS fungal infections. Coadministration w/ terfenadine, astemizole, cisapride, or oral triazolam.
Special Precautions
Patients requiring prolonged therapy or w/ history of liver disease. Measure LFTs eg, alkaline phosphatase, ALT, AST & bilirubin before starting & during treatment. Concomitant use w/ hepatotoxic drugs; antacids, anticholinergics & H2-blockers. May impair ability to drive & use machines. Hepatic impairment. Pregnancy. Should not breastfeed while on treatment. Childn <2 yr.
Adverse Reactions
Pruritus; nausea/vomiting, abdominal pain.
Drug Interactions
Inhibited absorption w/ antacids, H2 antagonists. Reduced therapeutic effects w/ sucralfate. Decreased bioavailability w/ PPI, INH. Decreased serum levels w/ rifampicin. Decreased theophylline serum levels. Increased plasma levels of cyclosporine, tacrolimus, methylprednisolone, midazolam, triazolam, digoxin, hypoglycemics, indinavir, ritonavir, saquinavir, TCAs, carbamazepine, quinidine, sulfonylureas, buspirone, HMG-CoA reductase inhibitors, nisoldipine, zolpidem, warfarin, corticosteroids & drugs metabolized by cytochrome P450 3A4 enzymes. Inhibited metabolism of terfenadine, astemizole & cisapride.
