Severe renal impairment CrCl <10 mL/min Not more than one 375-mg tab 12 hrly.
Susp Mild to moderate infections Childn 7-12 yr 10 mL of 228.5 mg/5 mL bid or 5 mL of 457 mg/5 mL bid,
2-6 yr 5 mL of 228.5 mg/5 mL bid or 2.5 mL of 457 mg/5 mL bid.
More serious infections Childn 7-12 yr 10 mL of 457 mg/5 mL bid,
2-6 yr 10 mL of 228.5 mg/5 mL bid or 5 mL of 457 mg/5 mL bid.
Childn 2 mth to <2 yr Individualized dose based on body wt.
Mild to moderate infections 25/3.6 mg/kg daily.
More serious infections 45/6.4 mg/kg daily.
