Fluvoxamine maleate
Indications/Uses
Major depressive episode, obsessive compulsive disorder (OCD).
Dosage/Direction for Use
Depression Recommended starting dose: 50 or 100 mg as single dose in the evening. May gradually increase dose until an effective dose is reached. Effective dose: 100 mg/day adjusted on individual patient response. Max: 300 mg/day. Prevention of recurrence of depression 100 mg daily. Obsessive compulsive disorder Adult Recommended starting dose: 50 mg/day for 3-4 days. May be increased gradually until effective dosage is achieved. Max: 300 mg/day. If total daily dose is >150 mg, administer in 2 or 3 divided doses. Adolescents & childn 8 yr Starting dose: 25 mg/day preferably at bedtime. May be increased every 4-7 days in 25 mg increments until effective dose is achieved. Max: 200 mg/day. If total daily dose is >50 mg, administer in 2 divided doses.
Administration
May be taken with or without food: Swallow whole w/ water, do not chew. Avoid high consumption of caffeine-containing beverages.
Contraindications
Hypersensitivity. Concomitant use w/ ramelteon, tizanidine & MAOIs.
Special Precautions
Renal & hepatic impairment. Patients w/ other psychiatric disorders & history of suicide-related events. Akathisia/psychomotor restlessness may develop. History of convulsive disorders, DM, bleeding disorder & mania/hypomania. Avoid in patients w/ unstable epilepsy; monitor carefully patients w/ controlled epilepsy; discontinue use if seizures occur. Patients w/ raised IOP or at risk of narrow-angle glaucoma, taking SSRIs particularly in elderly. Concomitant use w/ drugs known to affect platelet function (eg, atypical antipsychotics & phenothiazines, TCAs, acetylsalicylic acid, NSAIDs) or drugs that increase risk of bleeding; terfenadine, astemizole or cisapride; electroconvulsive therapy. Avoid abrupt w/drawal. May impair ability to drive or use machinery. Pregnancy & lactation. Childn <18 yr except for patients w/ OCD, elderly.
Adverse Reactions
Anorexia; agitation, nervousness, anxiety, insomnia, somnolence, tremor, headache, dizziness; palpitations, tachycardia; abdominal pain, constipation, diarrhoea, dry mouth, dyspepsia, nausea, vomiting; hyperhydrosis; asthenia, malaise.
Drug Interactions
Risk of serotonin syndrome w/ MAOIs including linezolid. May inhibit metabolism of CYP450 drugs. Increase AUC of ramelteon. Concomitant use w/ drugs w/ narrow therapeutic index (eg, tacrine, theophylline, methadone, mexiletine, phenytoin, carbamazepine & cyclosporine). Increased plasma levels of TCAs (eg, clomipramine, imipramine, amitriptyline) & neuroleptics (eg, clozapine, olanzapine, quetiapine), benzodiazepines (eg, triazolam, midazolam, alprazolam & diazepam), caffeine. Increase plasma conc of ropinirole, propranolol & warfarin. Increased risk of QT-prolongation/Torsade de Pointes w/ terfenadine, astemizole or cisapride. Enhanced serotonergic effects w/ other serotonergic agents (eg, triptans, tramadol, SSRIs & St. John's wort prep) & lithium. Increased risk of haemorrhage w/ oral anticoagulants. Alcohol.
