Adult & geriatricDVT/pulmonary embolism (PE) 10 mg bid for 7 days followed by 5 mg bid. Reduce risk of DVT/PE 2.5 mg bid after at least 6 mth of treatment. Prophylaxis of DVT 2.5 mg bid for 12-24 hr. Hip surgery For 35 days. Knee surgery For 12 days. Prevention of stroke & systemic embolism 5 mg bid, reduce dose to 2.5 mg bid if patient has 2 of the following: Age ≥80 yr, ≤60 kg or serum creatinine ≥1.5 mg/dL.
Increased risk of bleeding. Premature discontinuation increases risk of thrombotic events. Not recommended as alternative to unfractionated heparin for initial treatment of acute PE. Not recommended in patients w/ prosthetic heart valves or significant rheumatic heart disease. Childn <18 yr.
