Indications/Uses
VTE. Prevention of VTE in elective hip or knee replacement surgery; stroke & systemic embolism in non-valvular atrial fibrillation.
Dosage/Direction for Use
Adult & geriatric DVT/pulmonary embolism (PE) 10 mg bid for 7 days followed by 5 mg bid. Reduce risk of DVT/PE 2.5 mg bid after at least 6 mth of treatment. Prophylaxis of DVT 2.5 mg bid for 12-24 hr. Hip surgery For 35 days. Knee surgery For 12 days. Prevention of stroke & systemic embolism 5 mg bid, reduce dose to 2.5 mg bid if patient has 2 of the following: Age ≥80 yr, ≤60 kg or serum creatinine ≥1.5 mg/dL.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Active pathological bleeding.
Special Precautions
Increased risk of bleeding. Premature discontinuation increases risk of thrombotic events. Not recommended as alternative to unfractionated heparin for initial treatment of acute PE. Not recommended in patients w/ prosthetic heart valves or significant rheumatic heart disease. Childn <18 yr.
Adverse Reactions
Increased γ-glutamyl transferase & serum transaminase, gingival, postprocedural & rectal hemorrhage, nausea, hematuria, hypermenorrhea, anemia, bruise, hematoma, hemorrhage, epistaxis, hemoptysis.
Drug Interactions
Increased risk of bleeding w/ anticoagulants, antiplatelets, SNRIs, SSRIs, ticagrelor, nefazodone & grapefruit juice. Affected exposure w/ combined P-gp/strong CYP3A4. Increased exposure & risk for toxicity w/ fluconazole, itraconazole.
