Recombinant human erythropoietin α
Indications/Uses
Anemia of chronic renal failure (CRF) patients w/ symptomatic anemia & requiring blood transfusion. Reduction of transfusion requirements in patient receiving chemotherapy for solid tumor, malignant lymphoma or multiple myeloma & at risk of transfusion as assessed by the patients' general status. HIV-infected patients.
Dosage/Direction for Use
CRF patient Initially 50 u/kg 3 times wkly by slow SC or IV over 1 to approx 2 min, may be increased by 25 u/kg every 4 wk. Max dose: Not to exceed 200 u/kg in single day 3 times wkly. Cancer patient on chemotherapy Initially 150 u/kg SC 3 times wkly, may be increased up to 300 u/kg 3 times wkly. HIV-infected patient Initially 100 u/kg IV or SC 3 times wkly for 8 wk, may be increased by 50-100 u/kg 3 times wkly. Max dose: 300 u/kg 3 times wkly.
Contraindications
Hypersensitivity to mammalian cell-derived product or human albumin. Patients w/ uncontrolled HTN. History of pure red cell aplasia.
Special Precautions
Discontinue treatment in case of pure red blood cell aplasia. Not to be administered by IV infusion. Not to be administered to other anemias eg, dehematizing anemia, panhematopenia, Al storage disease. Patients w/ HTN; history of hypersensitivity; predisposition to allergy; MI, pulmonary & cerebral infarction; chance of thromboembolism. Premature w/ ventricular & intracerebral hemorrhage. Higher rate of seizure in patients on dialysis. Perform bone marrow testing in case reticulocyte index <20,000 microL. Regularly monitor Hb & hematocrit. Carefully monitor BP in patients w/ CV disease or chance of HTN. Proper dietary management in hyperkalemia. Monitor hemokinetic vol in shunt or hemodialysis kits. Concomitant use w/ interferon & ribavirin. Not to be used w/ any other erythropoietin prep. Not to be inj in conjunction w/ other drugs. Pregnancy. Childn. Elderly.
Adverse Reactions
Rise in BP; thrombosis of blood vessel; headache, consciousness disorder, seizure; encephalopathy; itch, rash, acne; hepatosis including AST, ALT, LDH, alkaline phosphatase; total bilirubin; nausea, vomiting, anorexia & diarrhea, abdominal pain; increased leukocyte, eosinophil, serum K, BUN, creatinine & uric acid, decreased granulocyte; eye-ground hemorrhage, splenomegaly, nasal hemorrhage, dizziness, pyrexia, slight fever, feeling of flush, malaise, arthralgia, myalgia, bitter taste of mouth, convulsion, blepharedema.
