Desloratadine
Indications/Uses
Rapid symptomatic relief of allergic rhinitis, eg, sneezing, nasal discharge & itching, congestion/stuffiness, as well as ocular itching, tearing & redness, palate itching & coughing. Symptomatic relief of urticaria eg, relief of itching & size & number of hives.
Dosage/Direction for Use
Symptomatic relief of allergic rhinitis (including intermittent & persistent allergic rhinitis) & urticaria Tab Adult & adolescent (≥12 yr) One 5-mg tab once daily. Syr Adult & adolescent (≥12 yr) 10 mL (5 mg). Childn 6-11 yr 5 mL (2.5 mg), 1-5 yr 2.5 mL (1.25 mg), 6-11 mth 2 mL (1 mg). All doses to be taken once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to desloratadine & loratadine.
Special Precautions
Consider discontinuing in patients who experience seizure while on treatment. Patients w/ medical or family history of seizures. Manage intermittent allergic rhinitis (presence of symptoms for <4 days/wk or for <4 wk) in accordance w/ patient's disease history evaluation & discontinue treatment after symptoms are resolved & reinitiated upon reappearance. Increased serum levels w/ macrolides (eg, erythromycin) or imidazole antifungal (eg, ketoconazole). May cause drowsiness in some patients, determine individual response before driving, operating machines, or working in areas w/ risk of falling as it may not cause drowsiness. Patients w/ renal disease. Severe renal insufficiency (GFR <30 mL/min). Not to be used during pregnancy especially in 1st trimester. Not recommended during lactation. Young childn may be more susceptible to developing new seizures. Childn <6 mth.
Adverse Reactions
Pharyngitis, nausea, somnolence, anorexia. Patient ≥12 yr, & ≥12 yr taking fixed combination w/ pseudoephedrine sulfate: Dry mouth, myalgia, insomnia, fatigue, somnolence, & dysmenorrhea; headache, dizziness, dyspepsia. Nervousness, hyperactivity. Ped 2-5 yr: UTI, & varicella (chicken pox); 12-23 mth: Increased appetite, emotional lability, epistaxis, parasitic infection, & maculopapular rash, fever, URTI, diarrhea, coughing; 6-11 mth: Irritability, bronchitis, otitis media, vomiting, rhinorrhea, erythema.
Drug Interactions
Increased plasma conc w/ drugs affecting hepatic microsomal enzymes eg, azithromycin, cimetidine, erythromycin, fluoxetine, ketoconazole.
