Indications/Uses
Management of acute pain. Short-term treatment of post-op pain.
Dosage/Direction for Use
Initially, 40 mg as IV/IM, followed by 20 or 40 mg 6- to 12-hrly as required. Max: 80 mg/day. Elderly <50 kg Initially ½ the usual dose. Max: 40 mg/day.
Contraindications
Hypersensitivity to parecoxib or sulfonamides. Patients w/ serious allergic-type reactions of any type, especially cutaneous reactions eg, SJS, TEN, erythema multiforme. Active peptic ulceration or GI bleeding, asthma, urticaria or allergic-type reactions after receiving aspirin or other NSAIDs eg, other COX-2 specific inhibitors. CHF (NYHA-II-IV). Severe hepatic dysfunction (serum albumin <25 g/L or Child-Pugh score ≥10). Inflammatory bowel disease. Post-op pain immediately following CABG surgery. Established ischemic heart disease, peripheral arterial disease &/or cerebrovascular disease. Pregnancy (3rd trimester) & lactation (3rd trimester).
Special Precautions
Increased CV & thrombotic adverse events in prolonged use. Patients w/ CV disease, using concomitant aspirin, corticosteroids, SSRIs or other NSAIDs, alcohol intake, or w/ history of, or active, GI disease eg, ulceration, bleeding, or inflammatory conditions & elderly may develop GI complications in concomitant use w/ NSAIDs. W/ or w/o known history of sulfonamide allergy. Discontinue at the 1st appearance of skin rash, mucosal lesions or any other sign of hypersensitivity. Severe hypotension may occur. Concomitant use w/ oral anticoagulants. HTN. Monitor BP closely during initiation of & throughout course of therapy. Compromised cardiac function, preexisting edema or other conditions pre-disposing to or worsened by fluid retention including patients taking diuretics or at risk of hypovolemia. Closely monitor renal function in patients w/ advanced renal disease. Rehydrate patients prior to & at initiation of therapy. Moderate & severe hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Nausea. Abdominal pain, constipation, dyspepsia, vomiting; peripheral edema; alveolar osteitis (dry socket); dizziness; insomnia; oliguria; increased sweating, pruritis; hypotension.
Drug Interactions
Increased AUC w/ CYP3A4 & 2C9 inhibitors, fluconazole & ketoconazole. May diminish effect of ACE inhibitors, AIIA, β-blockers & diuretics. May reduce natriuretic effect of furosemide & thiazides. Increased risk of nephrotoxicity w/ cyclosporine. Increase plasma levels of MTX. Decrease serum & renal clearance of lithium.
