Clobetasol propionate
Indications/Uses
Relief of inflammatory & pruritic manifestations of steroid responsive dermatoses (including the scalp) (eg, psoriasis, recalcitrant dermatoses, lichen planus, discoid lupus erythematosus & other skin conditions which do not respond satisfactorily to less potent steroids) in adults, elderly & childn >1 yr.
Dosage/Direction for Use
Cream/oint Apply thinly & gently rub in once daily or bid for up to 4 wk until improvement occurs, then reduce frequency or change treatment to less potent prep. Max wkly dose: 50 g. Recalcitrant dermatoses Patients who frequently relapse May apply as intermittent dosing (once daily, twice wkly, w/o occlusion) once acute episode has been treated effectively w/ continuous course of topical corticosteroid. Continue application to all previously affected sites or known sites of potential relapse & combined w/ routine daily use of emollient. Scalp application Apply bid until improvement occurs. May apply once daily or less frequently to sustain improvement.
Contraindications
Dermatoses in childn <1 yr, including dermatitis. Cream/Oint: Untreated cutaneous infections; rosacea; acne vulgaris; pruritus w/o inflammation, perianal & genital pruritus; perioral dermatitis. Scalp application: Scalp infection.
Special Precautions
History of local hypersensitivity. W/draw treatment gradually if manifestations of hypercortisolism (Cushing's syndrome) & reversible hypothalamic-pituitary-adrenal axis suppression, leading to glucocorticosteroid insufficiency are observed. Avoid smoking or being near to naked/open flames. Visual disturbance. Consider evaluation of cataract, glaucoma or central serous chorioretinopathy. Infection risk w/ occlusion; concomitant infection. Application to large surface area; use on occluded areas of skin (eg, intertriginous areas or under occlusive dressings), thin skin areas (eg, face), & broken skin or other conditions where skin barrier may be impaired; increased hydration of stratum corneum. Use in psoriasis as rebound relapses; development of tolerances & local or systemic toxicity, risk of generalised pustular psoriasis. Pregnancy & lactation. Infant & childn (more susceptible to systemic adverse effects & develop atrophic changes). Avoid long-term continuous topical corticosteroid therapy in infant & childn <12 yr. Cream/oint: Chronic leg ulcers. Application to eyelids. Serious fire hazard in fabrics in contact w/ the product.
Adverse Reactions
Drug Interactions
Increased systemic exposure w/ CYP3A4 inhibitors eg, ritonavir & itraconazole.
