Indications/Uses
Biliary tract infections due to E. coli, various strains of Strep, Proteus mirabilis, Klebsiella spp, & Staph aureus. Bone & joint infections due to Staph aureus. Resp tract infections due to Strep pneumoniae, Klebsiella spp, H. influenzae, Staph aureus (penicillin-sensitive & -resistant), & group A β-hemolytic Strep. Septicemia due to Staph aureus (penicillin-sensitive & -resistant). Skin & skin structure infections due to Strep pneumoniae, Staph aureus (penicillin-sensitive & -resistant), group A β-haemolytic Strep, and other strains of Strep. UTI due to E. coli, Proteus mirabilis, Klebsiella spp. Peri-operative prophylaxis.
Dosage/Direction for Use
IM/IV Inj deep IM into large muscle mass. Inj direct IV over 3-5 min or as intermittent infusion over 30-60 min. Adult & geriatric Mild infections caused by susceptible gm +ve cocci 250-500 mg every 8 hr. Moderate to severe infections 500 mg to 1 g every 6-8 hr. Severe, life-threatening infections (eg, endocarditis, septicemia) 1-1.5 g every 6 hr. May use up to 12 g daily in rare instances. Pneumococcal pneumonia 500 mg every 12 hr. Uncomplicated UTI 1 g every 12 hr. Group B Strep (perinatal prophylaxis) 2 g IV at the time of onset of labor or rupture of membranes followed by 1 g IV every 8 hr until delivery. Perioperative prophylaxis 1-2 g administer 30 min to 1 hr prior to start of surgery. Procedures ≥2 hr: 500 mg to 1 g during surgery; 24 hr post-op: 500 mg to 1 g every 6-8 hr. In surgery where infection may be particularly devastating (eg, open-heart, prosthetic arthroplasty): May continue prophylactic administration for 3-5 days following the completion of surgery. Childn >1 mth 25-50 mg/kg daily divided in 3 or 4 doses. Severe infections May be increased to 100 mg/kg daily divided in 3 or 4 doses. Max: 6 g daily. Renal impairment: Adult w/ CrCl 35-54 mL/min Administer full dose in ≥8-hr interval, 11-34 mL/min Administer 50% of usual dose every 12 hr, ≤10 mL/min Administer 50% of usual dose every 18-24 hr. Ped w/ CrCl >70 mL/min No dosage adjustment necessary, 40-70 mL/min 60% of usual daily dose divided every 12 hr, 20-40 mL/min 25% of usual daily dose divided every 12 hr, 5-20 mL/min 10% of usual daily dose divided every 24 hr.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Discontinue therapy if neurotoxicity develops; patient develops anemia any time w/in 2-3 wk after starting the therapy. Penicillin-sensitive patients. Erythema multiforme of skin rashes accompanied by polyarthritis, arthralgia, & fever. Severe neurological reactions including encephalopathy, myoclonus, seizures, & nonconvulsive status epilepticus. Increased INR especially in nutritionally-deficient patients, prolonged treatment, or hepatic or renal disease. Clostridium difficile-associated diarrhea (CDAD). Granulocytopenia & more rarely agranulocytosis during prolonged treatment (>10 days). Perform careful medical history as CDAD may occur >2 mth after administration of antibacterials. Periodically monitor patients in prolonged treatment for signs & symptoms of hemolytic anemia including measurement of hematological parameters. Monitor renal function prior to & during therapy in elderly & patients w/ known or suspected renal impairment. Renal impairment. Lactation.
Adverse Reactions
Localized phlebitis; seizure; pruritus, skin rash, SJS; abdominal cramps, anorexia, diarrhea, nausea, oral candidiasis, Pseudomembranous colitis, vomiting; vaginitis; hepatitis, increased serum transaminases; eosinophilia, leucopenia, neutropenia, thrombocythemia, thrombocytopenia; anaphylaxis; inj site pain; increased BUN & serum creatinine, renal failure; fever.
Drug Interactions
Decreased effectiveness of live vaccines. Acute alcohol intolerance (disulfiram-like reaction) w/ ethanol or alcoholic beverages. Diminished therapeutic effect of Na picosulfate. Enhanced anticoagulant effect of vit K antagonists. Increased risk of nephrotoxicity w/ potent diuretics eg, loop diuretics. Increased & prolonged plasma levels w/ probenecid.
