Betamethasone 17-valerate
Indications/Uses
Cream: Relief of inflammatory & pruritic manifestations of steroid-responsive dermatoses including atopic (infantile atopic dermatitis), nummular (discoid eczema), seborrheic, irritant or allergic contact dermatitis; prurigo nodularis; psoriasis (excluding widespread plaque psoriasis); lichen simplex chronicus (neurodermatitis) & planus; discoid lupus erythematosus; insect bite reactions, miliaria (prickly heat) & adjunct to systemic steroid therapy in generalized erythroderma in adult, elderly & childn >1 yr. Scalp application: Steroid-responsive dermatoses of scalp eg, psoriasis (excluding widespread plaque psoriasis), seborrhoea capitis, inflammation associated w/ severe dandruff.
Dosage/Direction for Use
Cream Adult, elderly & childn >1 yr Apply thinly to affected area once or twice daily for up to 4 wk. Patient who frequently relapse Intermittent dosing (once daily, twice wkly) once acute episode has been treated effectively. Scalp application Apply bid (morning & night). After condition has improved, apply once daily or less frequently.
Contraindications
Dermatoses including dermatitis in infants <1 yr. Cream: Untreated cutaneous infections; rosacea; acne vulgaris; pruritus w/o inflammation; perianal & genital pruritus; perioral dermatitis. Scalp application: Scalp infection.
Special Precautions
W/draw gradually if manifestations of hypercortisolism (Cushing's syndrome) & reversible hypothalamic-pituitary-adrenal (HPA) axis suppression are observed; in any spread of infection & administer appropriate antimicrobial therapy. Avoid abrupt w/drawal. Increased risk of systemic effects. Visual disturbance including cataract, glaucoma or central serous chorioretinopathy. Patients w/ psoriasis; history of local hypersensitivity to corticosteroids. Avoid contact w/ eyes. Clean skin before applying fresh occlusive dressing. Do not smoke or use near naked flames. Renal/hepatic impairment. Pregnancy & lactation. Adrenal suppression in infant & childn <12 yr; avoid long-term, continuous use. Elderly. Cream: Higher occurrence of local hypersensitivity reactions & increased risk of local infection when used to treat dermatitis around chronic leg ulcers. Prolonged application to face.
Adverse Reactions
Drug Interactions
Increased systemic exposure w/ CYP3A4 inhibitors eg, ritonavir, itraconazole.
