Indications/Uses
Susceptible bacterial infections eg, lower resp tract infections (bronchitis, pneumonia), URTI (sinusitis, pharyngitis/tonsilitis), ondontostomatological infections, acute otitis media, skin & soft tissue infections, STD (uncomplicated genital infection due to Chlamydia trachomatis, chancroid due to Haemophilus ducreyi, uncomplicated genital infections due to non-multiresistant Neisseria gonorrhoeae, disseminated Mycobacterium avium intracellulare complex (DMAC) infection in patients w/ advanced HIV infection). Prophylaxis against Mycobacterium avium intracellulare complex (MAC).
Dosage/Direction for Use
Cap Adult STD caused by Chlamydia trachomatis & Haemophilus ducreyi 1,000 mg as single dose. Susceptible Neisseria gonorrhoeae 1,000 mg or 2,000 mg in combination w/ 250 mg or 500 mg ceftriaxone. DMAC infection in patients w/ advanced HIV 600 mg once daily in combination w/ ethambutol. All other indication Total daily dose: 1,500 mg given as 500 mg once daily for 3 days. Alternatively, 500 mg on day 1, then 250 mg daily on days 2-5. Community acquired pneumonia 500 mg once daily to complete 7-10 day course following IV therapy. Pelvic inflammatory diseases 250 mg once daily to complete 7-day course following IV therapy. MAC prophylaxis in HIV-infected patient 1,200 mg once a wk. Childn weighing >45 kg Total daily dose: 30 mg/kg given as 10 mg/kg daily for 3 days or 10 mg/kg on day 1, then 5 mg/kg daily on days 2-5. Max recommended dose: 1,500 mg. Susp All doses given on 3-day regimen. Childn weighing >45 kg Follow adult dose. Total dose: 1,500 mg, 36-45 kg 400 mg (10 mL) once daily. Total dose: 1,200 mg, 26-35 kg 300 mg (7.5 mL) once daily. Total dose: 900 mg, 15-25 kg 200 mg (5 mL) once daily. Total dose: 600 mg, <15 kg 10 mg/kg once daily. Total dose: 600 mg, Ped streptococcal pharyngitis in patients contraindicated w/ penicillin 10 mg/kg as single dose or 20 mg/kg for 3 days. Max daily dose: 500 mg. Acute otitis media Childn 30 mg/kg as single dose.
Administration
Should be taken on an empty stomach: Do not take w/ Al- & Mg-containing antacids.
Contraindications
Hypersensitivity to azithromycin, erythromycin, macrolides or ketolide antibiotic.
Special Precautions
Discontinue if allergic reaction, signs & symptoms of hepatitis occur. Superinfection; Clostridium difficile-associated diarrhea; prolonged cardiac repolarization & QT interval; exacerbation of myasthenia gravis symptoms. Patients w/ congenital or documented QT prolongation; electrolyte disturbance, particularly in hypokalaemia & hypomagnesemia; clinically relevant bradycardia, cardiac arrhythmia or insufficiency. Not to be co-administered w/ ergot derivatives. Concomitant treatment w/ other QT-prolonging substances eg, class IA & III antiarrhythmics, antipsychotics, antidepressants & fluoroquinolones. Hepatic disease. Severe renal impairment (GFR <10 mL/min). Pregnancy & lactation. Infantile hypertrophic pyloric stenosis in neonates. Elderly.
Adverse Reactions
Moniliasis, vaginitis; thrombocytopenia; anaphylaxis; anorexia; aggressive reaction, nervousness, agitation, anxiety; dizziness, convulsions, headache, hyperactivity, hypoesthesia, paresthesia, somnolence, syncope; deafness, tinnitus, impaired hearing, vertigo; palpitations & arrhythmias including ventricular tachycardia; hypotension; vomiting/diarrhea, dyspepsia, constipation, pseudomembranous colitis, pancreatitis; abnormal liver function, hepatitis, cholestatic jaundice; allergic reactions including pruritus, rash, photosensitivity, edema, urticaria, angioedema; arthralgia; interstitial nephritis, acute renal failure; local pain & inflammation at infusion site, asthenia, fatigue, malaise. Erythema multiforme, acute generalized exanthematous pustulosis, SJS, TEN & DRESS.
Drug Interactions
Decreased absorption rate w/ Al- & Mg hydroxide-containing antacids. Increased plasma conc of cyclosporine; pimozide Increased serum levels of digoxin. Ergotism w/ ergot derivatives. Rhabdomyolysis w/ atorvastatin. Potentiated anticoagulation w/ coumarin-type oral anticoagulants. Increased conc of phosphorylated zidovudine or its glucuronide metabolite.
