Adult & childn >40 kg 250 mg tid, may be increased to 500 mg tid for more severe infections.
Severe or recurrent purulent resp tract infection 3 g bid. Max: 6 g daily in divided doses.
Simple acute UTI Two 3-g doses every 10-12 hr.
Dental abscess Two 3-g doses every 8 hr.
Gonorrhoea 3 g as single dose.
H. pylori eradication Amoxicillin 750 mg-1 g bid in combination w/ PPI (omeprazole or lansoprazole) & another antibiotic (clarithromycin or metronidazole) for 7 days.
Prophylaxis of endocarditis in patients requiring manipulation of gingival or periapical teeth region or perforation of oral mucosa 2 g as single oral dose 30-60 min before dental procedure.
Childn <40 kg 125 mg tid, may increase to 250 mg tid for more severe infections.
Acute otitis media 750 mg bid for 2 days as alternative course of treatment.

Renal impairment in adult & childn >40 kg w/ CrCl 10-30 mL/min (moderate) Max: 500 mg bid;
CrCl <10 mL/min (severe) Max: 500 mg daily,
childn <40 kg w/ CrCl 10-30 mL/min (moderate) 15 mg/kg bid (max: 500 mg bid);
CrCl <10 mL/min (severe) 15 mg/kg once daily (max: 500 mg).
Patient receiving peritoneal dialysis Max: 500 mg daily.
Patient receiving haemodialysis: Adult 500 mg as 1 additional dose,
childn <40 kg 15 mg/kg as 1 additional dose.
