Amoxicillin trihydrate
Indications/Uses
Infections of upper resp tract (eg, ENT infections, otitis media), lower resp tract (eg, acute exacerbations of chronic bronchitis, lobar & bronchopneumonia), GIT (eg, typhoid & paratyphoid fever), GUT (eg, cystitis, urethritis, pyelonephritis, bacteriuria in pregnancy, septic abortion, puerperal sepsis), skin & soft tissue, biliary tract, bone, pelvic; borreliosis or Lyme disease (Borrelia burgdorferi); gonorrhoea (non-penicillinase-producing strains), septicaemia, endocarditis, meningitis, peritonitis. H. pylori eradication in PUD (duodenal & gastric). Adjunct to surgical management of dental abscess. Prophylaxis of endocarditis.
Dosage/Direction for Use
Adult & childn >40 kg 250 mg tid, may be increased to 500 mg tid for more severe infections. Severe or recurrent purulent resp tract infection 3 g bid. Max: 6 g daily in divided doses. Simple acute UTI Two 3-g doses every 10-12 hr. Dental abscess Two 3-g doses every 8 hr. Gonorrhoea 3 g as single dose. H. pylori eradication Amoxicillin 750 mg-1 g bid in combination w/ PPI (omeprazole or lansoprazole) & another antibiotic (clarithromycin or metronidazole) for 7 days. Prophylaxis of endocarditis in patients requiring manipulation of gingival or periapical teeth region or perforation of oral mucosa 2 g as single oral dose 30-60 min before dental procedure. Childn <40 kg 125 mg tid, may increase to 250 mg tid for more severe infections. Acute otitis media 750 mg bid for 2 days as alternative course of treatment. Prophylaxis of endocarditis in patients requiring manipulation of gingival or periapical teeth region or perforation of oral mucosa 50 mg/kg as single oral dose 30-60 min before dental procedure. Renal impairment in adult & childn >40 kg w/ CrCl 10-30 mL/min (moderate) Max: 500 mg bid; CrCl <10 mL/min (severe) Max: 500 mg daily, childn <40 kg w/ CrCl 10-30 mL/min (moderate) 15 mg/kg bid (max: 500 mg bid); CrCl <10 mL/min (severe) 15 mg/kg once daily (max: 500 mg). Patient receiving peritoneal dialysis Max: 500 mg daily. Patient receiving haemodialysis: Adult 500 mg as 1 additional dose, childn <40 kg 15 mg/kg as 1 additional dose.
Administration
May be taken with or without food: May be taken w/ meals for better absorption & to reduce GI discomfort.
Contraindications
History of hypersensitivity to β-lactam antibiotics eg, penicillins or cephalosporins.
Special Precautions
Serious & occasionally fatal hypersensitivity reactions (including anaphylactoid & severe cutaneous adverse reaction). History of hypersensitivity to β-lactam antibiotics. Discontinue in case of erythema, rash, irritation or edema, flu-like symptoms, fever, vesicles, exfoliation of skin & other mucous membranes eg, mouth, throat, nose, reproductive organ & conjunctivitis; if allergic reaction, prolonged or significant diarrhoea or abdominal cramps occur. Avoid if infectious mononucleosis is suspected. Kounis syndrome; pseudomembranous colitis; abnormal prothrombin time prolongation (increased INR). Overgrowth of non-susceptible organisms w/ prolonged use. Patients w/ reduced urine output; phenylketonuria. Maintain adequate fluid intake & urinary output. Concomitant use w/ oral anticoagulants. Renal impairment. May increase jaundice in newborn babies (up to 4 wk old).
Adverse Reactions
Diarrhea, nausea; skin rash. Erythema multiforme, SJS, TEN, DRESS.
Drug Interactions
Decreased renal tubular secretion w/ probenecid. Increased allergic reactions w/ allopurinol. False +ve readings w/ urine glucose test. Lower oestrogen reabsorption. Reduced efficacy of combined OCs. Increased INR w/ acenocoumarol or warfarin.
