Alfacalcidol
Indications/Uses
Conditions caused by disturbance of Ca metabolism due to impaired 1-α hydroxylation eg, chronic renal failure.
Dosage/Direction for Use
Initial dose: Adult & childn weighing >20 kg 1 mcg daily. Childn weighing <20 kg 0.05 mcg/kg daily. Elderly 0.5 mcg daily. Maintenance dose: 0.25-1 mcg daily. Daily doses may be increased by 0.25-0.5 mcg increments. Severe hypocalcemia 3-5 mcg daily w/ Ca supplements.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Hypercalcemia, metastatic calcification.
Special Precautions
Discontinue if hypercalcemia occurs. Patients treated w/ cardioactive glycosides or digitalis; w/ nephrolithiasis. Chronic hypercalcemia leading to generalized vascular calcification, nephrocalcinosis or calcifications of cornea or other soft tissues. Avoid prolonged use. Monitor serum Ca & phosphate regularly during treatment in childn, patients w/ renal impairment & receiving high doses. Use phosphate binding agent in patients w/ renal bone disease. Perform radiographic evaluation of suspect anatomical regions w/ long-term therapy. Determine serum Ca, P, Mg, alkaline phosphate, creatinine, BUN & plasma protein. Not to be used concomitantly w/ vit D or derivatives. Avoid Mg-containing antacids. Pregnancy & lactation.
Adverse Reactions
Hypercalcemia, hyperphosphatemia, various skin reactions.
Drug Interactions
Induction of hepatic detoxification enzymes w/ barbiturates or anticonvulsants. Interference of intestinal absorption w/ cholestyramine. Increased risk of hypercalcemia w/ thiazide diuretics.
