Fe sucrose
Indications/Uses
Fe deficiency where there is a clinical need for a rapid Fe supply; in patients who cannot tolerate oral Fe therapy or non-compliant; where oral Fe prep are ineffective (eg, active inflammatory bowel disease).
Dosage/Direction for Use
IV Individualized dosage. Adults 100-200 mg 1-3 times a wk. Childn Max: 3 mg/kg 3 times a wk. Inj Max tolerated daily dose: 200 mg over at least 10 min, to be given not more than 3 times a wk. Infusion Patient weighing >70 kg Max tolerated daily dose: 500 mg over at least 3 ½ hr, to be given not more than once a wk, ≤70 kg Max tolerated daily dose: 7 mg/kg over at least 3 ½ hr, to be given not more than once a wk.
Contraindications
Hypersensitivity. Anaemia not caused by Fe deficiency. Evidence of Fe overload or Fe utilisation disturbances. Pregnancy (1st trimester).
Special Precautions
Potentially fatal allergic or anaphylactoid reactions; risk of allergic reaction in patients w/ history of asthma, eczema, other atopic allergies or allergic reactions to other parenteral Fe prep. Observe for adverse effects for at least 30 min following each inj. Possible progression of hypersensitivity reactions to Kounis syndrome. Patients w/ acute or chronic infection; stop administration in patients w/ bacteraemia. Avoid paravenous leakage; immediately stop administration when pain, inflammation, tissue necrosis & brown skin discoloration occur. Patients w/ liver dysfunction. Carefully monitor Fe status to avoid Fe overload. Pregnancy (2nd & 3rd trimester). Lactation.
Adverse Reactions
Dysgeusia; hypotension, HTN; nausea; inj/infusion site reactions.
Drug Interactions
May reduce oral Fe prep absorption (start oral Fe therapy at least 5 days after last inj).
