Description
INGREDIENTS: Enzymatically hydrolysed whey protein (milk), maltodextrin, medium chain triglycerides, minerals (calcium glycerophosphate, calcium phosphate, magnesum chloride, calcium chloride, sodium citrate, sodium chloride, ferrous sulphate, zinc sulphate, copper sulphate, potassium iodide, manganese sulphate, sodium selenite), vegetable oil, glucose syrup, vitamins [sodium ascorbate (vit C), dl-alpha tocopheryl acetate (vit E), niacinamide (niacin), calcium pantothenate, retinyl acetate (vit A), riboflavin (vit B2), thiamine mononitrate (vit B1), pyridoxine hydrochloride (vit B6), folic acid, phylloquinone (vit K), cholecalciferol (vit D), biotin, cyanocobalamine (vit B12)], fish oil, choline bitartrate, acidity regulator (citric acid), inositol, L-carnitine, taurine, antioxidant (mixed tocopherols concentrate).

Conveniently packaged in 1g sachets: one sachet will fortify 25mL of human milk.

Dosage/Direction for Use
FEEDING GUIDE: PreNAN HMF is a food for special medical purposes especially designed to support the growth of premature/low birth weight infants fed with human milk.
The following is given as a guide and may be changed based on medical recommendation. An amount appropriate for the weight and age of the premature/low birth weight infants should be prescribed.
The standard level of fortification is 1 g per 25 ml of human milk. It is recommended to proceed gradually, starting with 2 g of PreNAN HMF per 100 ml of human milk.
After evaluating tolerance over a 24 hour period, fortification can be advanced to full strength (4 g of PreNAN HMF per 100 ml of human milk) or follow hospital guidelines on feeding.
Contraindications
PreNAN Human Milk Fortifier is not a complete food. PreNAN Human Milk Fortifier is not suitable for use in infants with diagnosed cow's milk protein allergy, lactose intolerance and metabolic congenital diseases such as galactosaemia/glucose-galactose malabsorption. Not intended for parenteral use.

Warnings
Always wash hands and sterilize feeding devices before preparing baby's feed. Only prepare one feed at a time. Feed immediately and follow instructions exactly. Incorrect dilution can make baby ill. Discard all unfinished quantities of fortified milk after each feed. For hospital use only, under medical supervision.
Special Precautions
IMPORTANT NOTICE: Human milk is best for most premature and low-birth-weight infants.
REMINDER: Always hold baby while feeding. Leaving baby unattended may cause choking.
Caution For Usage
FEED PREPARATION: 1. Each feeding device (bottle, syringe or container for tube feed) must be prepared individually just before feeding.
2. If frozen human milk is used, gradually defrost the
human milk in refrigerator (under 4°C). DO NOT use a microwave oven or leave it at room temperature to defrost.
3. Shake or swirl before pouring into the feeding device.
4. Pour only required amount of expressed human milk to be fortified into the feeding device.
5. Before mixing with fortifier, warm up human milk to body temperature. DO NOT use a microwave oven.
6. Add the required quantity of PreNAN HMF to desired quantity of lukewarm human milk (35°C-40°C), using the standard preparation (1 g for 25 ml of human milk) or a specifically calculated quantity if a different reconstitution is prescribed.
7. Cap and gently shake until powder is fully dissolved and FEED IMMEDIATELY.
8. Discard all unfinished quantities of fortified human milk after each feed.
9. Opened sachet should be used immediately or discarded.
10. For tube feeding: once fortified human milk is prepared, it should be used within 4 hours.
