Indications/Uses
Dosage/Direction for Use
Adult Recommended dose: 50 mg 1 hr before sexual activity, increased to 100 mg or decreased to 25 mg. Max recommended dose & frequency: 100 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Co-administration w/ nitric oxide donors (eg, amyl nitrite) or nitrates in any form. Men for whom sexual activity is inadvisable [eg, patients w/ severe CV disorders (eg, unstable angina or severe cardiac failure)]. Non-arteritic anterior ischaemic optic neuropathy (NAION). Severe hepatic impairment, hypotension (BP <90/50 mmHg), recent history of stroke or MI & known hereditary degenerative retinal disorders eg, retinitis pigmentosa. Women. Childn <18 yr.
Special Precautions
Thorough medical history & physical examination should be undertaken to diagnose erectile dysfunction, determine potential underlying causes & identify appropriate treatment. Should not be used in men for whom sexual activity is inadvisable. Priapism. Co-administration of an α-blocker may lead to symptomatic hypotension. Patients w/ retinitis pigmentosa; bleeding disorders or active peptic ulceration; anatomical deformation of the penis (eg, angulation, cavernosal fibrosis or Peyronie's disease) or in patients who have conditions which may predispose them to priapism (eg, sickle cell anemia, multiple myeloma or leukemia); resting HTN (BP >170/110), life-threatening arrhythmia w/in the last 6 mth & history of cardiac failure or CAD causing unstable angina. Discontinue use if sudden decrease or loss of hearing occur. May affect ability to drive or operate machinery.
Adverse Reactions
Headache, flushing; dizziness, abnormal vision (eg, blurred vision, increased sensitivity to light), chromatopsia (mild & transient, predominantly color tinge to vision); palpitation; rhinitis, nasal congestion; dyspepsia.
Drug Interactions
Reduced clearance w/ CYP3A4 inhibitors (eg, ketoconazole, erythromycin, cimetidine). Increased Cmax & AUC w/ saquinavir, ritonavir. Increased plasma levels w/ grapefruit juice. Potentiate the hypotensive effect of acute & chronic nitrates (eg, nitric oxide donors, organic nitrates & nitrites).
