Indications/Uses
Relieve w/drawal symptoms & reduce cravings for nicotine during smoking cessation.
Dosage/Direction for Use
Stop smoking completely during treatment. Monotherapy Nicorette Invisi Transdermal Patch Heavy smoker (≥15 cigarettes in a 24-hr period) Initially apply one 25 mg patch daily to the skin in the morning & remove 16 hr later at bedtime for 8 wk, followed by one 15 mg patch daily for 2 wk & then one 10 mg patch daily for 2 wk. Light smoker (<15 cigarettes in a 24-hr period) Initially apply one 15 mg patch daily to the skin in the morning & remove 16 hr later at bedtime for 8 wk, followed by one 10 mg patch daily for 4 wk. Nicorette Icy Mint Medicated Chewing Gum Heavy smoker (>20 cigarettes daily) 4 mg gum daily. Usual dose: 8-12 gum daily for 12 wk then 4-6 gum daily for 2 wk & reduce to 1-3 gum daily (& gradually reduce to 0) for 2 wk. Max daily dose: 24 gum. Light smoker (≤20 cigarettes daily) 2 mg gum daily. Usual dose: 8-12 gum daily for 12 wk then 4-6 gum daily for 2 wk & reduce to 1-3 gum daily (& gradually reduce to 0) for 2 wk. Max daily dose: 30 gum. Combination therapy Nicorette Invisi Transdermal Patch w/ Nicorette Icy Mint Medicated Chewing Gum Heavy smoker (>15 cigarettes in a 24-hr period) Apply one 25 mg patch & take 2 mg gum (at least 4 gum; max: 12 gum) daily for 12 wk. Light smoker (<15 cigarettes in a 24-hr period) Apply one 15 mg patch & take 2 mg gum (at least 4 gum; max: 12 gum) daily for 12 wk. Nicorette QuickMist Mouth Spray Wk 1-6: Use 1-2 sprays when the patient would normally smoke a cigarette or have cravings to smoke. If 2 sprays are required, future doses may be delivered as 2 consecutive sprays. Usual dose: 1-2 sprays every 30 min to 1 hr. Max: 2 sprays at a time, 3 sprays/hr & 48 sprays daily. Wk 7-9: Reduce number of sprays daily until the patient is using ½ the ave number of sprays daily in wk 1-6. Wk 10-12: Continue reducing number of sprays daily until the patient is not using >3 sprays daily during wk 12.
Administration
May be taken with or without food: Chew gum slowly until the taste becomes strong (approx 1 min), then rest it between the gums & the cheek (1 min). When the taste fades, start chewing it again (about 20-30 min). Do not swallow.
Contraindications
Special Precautions
Patient w/ recent MI, unstable or worsening angina including Prinzmetal's angina, severe cardiac arrhythmias, uncontrolled HTN; uncontrolled hyperthyroidism & phaeochromocytoma; history of epilepsy or seizures. Moderate to severe hepatic &/or severe renal impairment. Pregnancy. Avoid use during breast-feeding. Nicorette Invisi Transdermal Patch & Nicorette Icy Mint Medicated Chewing Gum Combination therapy of 4 mg Nicorette Icy Mint Medicated Chewing Gum & Nicorette Invisi Transdermal Patch should not be instituted. Patients w/ CV disease; recent CVA. Closely monitor blood sugar levels in patients w/ DM. May exacerbate symptoms in patients suffering from oesophagitis, gastric or peptic ulcers. Transferred dependence. Adolescent <18 yr. Potentially fatal, severe toxicity in childn. Nicorette Invisi Transdermal Patch Remove prior to MRI procedures to prevent risk of burns. Nicorette Icy Mint Medicated Chewing Gum May experience chewing difficulty in smokers who wear dentures; may damage dentures. Nicorette QuickMist Mouth Spray Allergic reactions that involve lips swelling, angioedema or urticaria; diabetes; overactive thyroid gland; stomach ulcer; oesophagitis. Not to be used by non-smokers. Do not eat or drink when administering the mouth spray. Contains small amounts of ethanol (<100 mg/spray). Do not use in childn <18 yr.
Adverse Reactions
Nausea, vomiting; fatigue; hypersensitivity; headache. Nicorette Invisi Transdermal Patch & Nicorette Icy Mint Medicated Chewing Gum Paraesthesia. Nicorette Icy Mint Medicated Chewing Gum & Nicorette QuickMist Mouth Spray Dry mouth; burning sensation; cough, hiccups, throat irritation. Nicorette Invisi Transdermal Patch Pruritus. Nicorette Icy Mint Medicated Chewing Gum Abdominal pain, dyspepsia, flatulence, salivary hypersecretion, stomatitis; dysgeusia. Nicorette QuickMist Mouth Spray Inflammation/pain in mouth or lips, taste perception & saliva production changes, indigestion; dizziness, sore mouth, tingling/numbness of hands or feet, stomach pain/discomfort, diarrhoea, wind.
Drug Interactions
Concomitant use w/ products w/ narrow therapeutic window (eg, theophylline, tacrine, clozapine & ropinirole). Nicorette Invisi Transdermal Patch & Nicorette Icy Mint Medicated Chewing Gum May possibly enhance haemodynamic effects of adenosine ie, increase in BP & heart rate, & also increased pain response (angina pectoris type chest pain) provoked by adenosine administration. Nicorette QuickMist Mouth Spray Concomitant use w/ flecainide & pentazocine.
