Losartan K
Indications/Uses
Alone or in combination w/ other antihypertensive agents, including diuretics, for HTN. Reduce the risk of stroke in patients w/ HTN & left ventricular hypertrophy.
Dosage/Direction for Use
HTN Usual starting & maintenance dose: 50 mg once daily, may be increased to 100 mg once daily. Patient w/ intravascular vol depletion (eg, those treated w/ high-dose diuretics) Initially 25 mg once daily. Patient >75 yr, moderate to severe renal impairment, & on dialysis Initially 25 mg. Reduction in the risk of stroke in hypertensive patient w/ left ventricular hypertrophy Initially 50 mg once daily. Add a low dose of hydrochlorothiazide &/or increase losartan K dose to 100 mg once daily based on BP response.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Co-administration w/ aliskiren in patients w/ diabetes or w/ renal impairment (GFR <60 mL/min). Pregnancy (2nd & 3rd trimester).
Special Precautions
Symptomatic hypotension may occur in patients w/ intravascular vol depletion (eg, those treated w/ diuretics). Electrolyte imbalance in patients w/ or w/o diabetes. Patients whose renal function may depend on the activity of the renin angiotensin aldosterone system (eg, w/ severe CHF); w/ unilateral or bilateral renal artery stenosis. Hepatic impairment. Not for concomitant use w/ K supplements or salt substitutes containing K. Lactation. Childn. Elderly >75 yr.
Adverse Reactions
Dizziness, asthenia/fatigue, vertigo, hypotension & hyperkalemia.
Drug Interactions
May have increased serum K w/ K-sparing diuretics (spironolactone, triamterene, amiloride), K supplements or salt substitutes containing K. May reduce lithium excretion. May have attenuated antihypertensive effect w/ NSAIDs including selective cyclooxygenase-2 inhibitors. Increased risk of hypotension, syncope, hyperkalemia & changes in renal function (including acute renal failure) w/ ACE inhibitors, or aliskiren.
