Ammonium lactate.
Description
Lanate also contains the following excipients: Glycerine, purified water, Polawax GP200 (a mixture of non-ionic blend of higher fatty alcohols and ethylene oxide condensation products), glycerol monostearate, Nipastat GL75 (mixture of methyl, ethyl, propyl and butyl hydroxybenzoate), silicone fluid, light paraffin oil and isopropyl myristate.
Lanate contains lactic acid neutralised with ammonium hydroxide. This produces a cream with a pH of 4-5.
Ammonium lactate is the ammonium salt of lactic acid. Its chemical formula is C3H9NO3 and its molecular weight is 107.109.
Action
The lactic acid produced from Lanate cream is a humectant on the skin and has beneficial effects when applied to dry skin, especially dry, scaly and lined skin.
Lactic acid, an α-hydroxy acid, is reported to be one of the most effective naturally occurring humectants in the skin. The α-hydroxy acids, in addition to having beneficial effects on dry skin, have also been shown to reduce excessive epidermal keratinization in patients with hyperkeratotic conditions (eg, ichthyosis). Lanate may also improve the appearance of rough skin such as that seen in keratosis pilaris.
Pharmacokinetics: The mechanism of action of topically applied neutralized lactic acid is not yet known.
Indications/Uses
Revitalizes the appearance of dry, scaly skin. Helps moisturize the skin through its humectant effect making skin feel and look smoother and more supple. Provides temporary relief from itching. Helps repair damaged skin.
Dosage/Direction for Use
Apply sparingly to the affected area of the skin and rub-in thoroughly twice daily.

Contraindications
Known hypersensitivity to any of the ingredients of Lanate.
Special Precautions
Sensitive skin may be more responsive to the exfoliating effects of Lanate, which may result in irritation. If the patient has sensitive skin, test Lanate on a small area of the skin before applying the cream to a larger area.
Transient stinging or irritation may occur when using Lanate. If severe stinging or excessive irritation occurs after application, wash the affected area and discontinue use.
Use in pregnancy: Pregnancy Category B: Animal reproduction studies performed in rats and rabbits at doses up to 0.7 and 1.5 times the human dose, respectively (600 mg/kg/day, corresponding to 3600 mg/m2/day in the rat and 7200 mg/m2/day in the rabbit) have revealed no evidence of impaired fertility or harm to the fetus due to ammonium lactate cream. Because there are no adequate and well-controlled studies in pregnant women, Lanate should be used during pregnancy only if clearly needed.
Use in lactation: Although lactic acid is a normal constituent of blood and tissues, it is not known to what extent this drug affects normal lactic acid levels in human milk. Because many drugs are excreted in human milk, caution should be exercised when Lanate is administered to a nursing mother.
Use in children: The safety and effectiveness of ammonium lactate preparations for external use has been demonstrated in infants and children.
Use In Pregnancy & Lactation
Use in pregnancy: Pregnancy Category B: Animal reproduction studies performed in rats and rabbits at doses up to 0.7 and 1.5 times the human dose, respectively (600 mg/kg/day, corresponding to 3600 mg/m2/day in the rat and 7200 mg/m2/day in the rabbit) have revealed no evidence of impaired fertility or harm to the fetus due to ammonium lactate cream. Because there are no adequate and well-controlled studies in pregnant women, Lanate should be used during pregnancy only if clearly needed.

Adverse Reactions
Patients with psoriasis may experience transient stinging, burning, erythema and peeling.
Although other rare adverse reactions eg, irritation, eczema, petechiae, dryness and hyperpigmentation may occur.
Drug Interactions
No clinically significant interactions have been reported.
Incompatibilities: No incompatibilities with other medicines have been recorded.
Caution For Usage
Lanate is for external use only. It is not for ophthalmic, oral or intravaginal use. Avoid contact with eyes, lips and mucous membranes. If the cream comes in contact with the eye, wash it out immediately with cool water.
Lanate is a high strength α-hydroxy acid cream and must be used with care to ensure best results.
Note: By removing the surface layer of dead skin cells, Lanate may make the skin more sensitive to sunlight especially to the ultraviolet (UV) radiation component of sunlight. As UV exposure can damage the skin, sun exposure should be limited by routinely using an effective sunscreen and by wearing protective clothing.
