Zolid Pioglitazone HCl. ZolidPlus Metformin HCl 500 mg, pioglitazone HCl 15 mg
Indications/Uses
Zolid 15 mg Management of Type 2 DM alone or in combination w/ metformin or a sulfonylurea. 30 mg Adjunct to diet & exercise to improve glycemic control in patients w/ NIDDM. Alone or used in combination w/ sulfonylurea, metformin or insulin when diet & exercise plus the single agent does not result in adequate glycemic control. ZolidPlus Type 2 DM when monotherapy is inadequate or for patients requiring combined or more intensive therapy.
Dosage/Direction for Use
Zolid Monotherapy 15 or 30 mg once daily. Combination therapy 15 or 30 mg once daily. Patient at risk of hypoglycemia or w/ hepatic impairment Max dose: 45 mg once daily. ZolidPlus Patient inadequately controlled on metformin monotherapy 15 mg/500 mg once or bid. Patient who initially responded to pioglitazone monotherapy & require additional glycemic control 15 mg/500 mg once or bid. Patient switching from combination therapy of pioglitazone plus metformin as separate tab 15 mg/500 mg once or bid.
Administration
May be taken with or without food.
Contraindications
Zolid Heart failure. IDDM (type I); treatment of diabetic ketoacidosis. ZolidPlus Cardiac failure or history of cardiac failure (NYHA class III or IV). Hepatic impairment or active liver disease, acute or chronic disease causing tissue hypoxia eg, cardiac or resp failure, recent MI, shock. Acute alcohol intoxication, alcoholism. Renal failure or dysfunction (CrCl <60 mL/min). Patients w/ type 1 DM or diabetic ketoacidosis.
Special Precautions
Zolid Hypoglycemia, edema, anemia. Postmenopausal women. Active liver disease or increased serum transaminase levels. Pregnancy & lactation. Childn. ZolidPlus Cardiac failure. Therapy w/ pioglitazone may result in ovulation in some premenopausal anovulatory women. Monitor Hb. Therapy should not be initiated in patients w/ acute liver disease or ALT levels exceeds 2.5 times the ULN. Lactic acidosis. Discontinue use when CV collapse (shock), acute CHF & MI occur. Avoid excessive alcohol intake. Hypoglycemia when caloric intake is deficient, strenuous exercise is not compensated by caloric supplementation or concomitant use w/ hypoglycemic agents (eg, sulfonylurea or insulin) or ethanol.
Adverse Reactions
Upper resp tract infections, headache. Zolid Sinusitis, myalgia, tooth disorder, DM aggravation & pharyngitis. ZolidPlus Anemia, visual disturbance, wt gain, arthralgia, hematuria, erectile dysfunction. Hypoesthesia; taste disturbance, nausea, vomiting, diarrhea, abdominal pain, loss of appetite.
Drug Interactions
Zolid Ketoconazole; OC containing ethinyl estradiol & norethindrone. ZolidPlus Pioglitazone: CYP2C8 inducers (eg, rifampin) & inhibitors (eg, gemfibrozil), OCs. Metformin HCl: Cationic drugs (eg, amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim, vancomycin); furosemide, nifedipine, thiazides & other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, phenytoin, nicotinic acid, sympathomimetics, Ca channel blockers & INH.
