Metronidazole
Indications/Uses
Treatment of anaerobic infections due to Bacteroides fragilis including those involving the CNS eg, meningitis, brain abscess, endocarditis, intra-abdominal infections, sepsis. Treatment of antibiotic-induced pseudomembranous colitis due to C. difficile bacterial vaginosis (due to Gardnerella vaginalis). Prophylaxis of anaerobic infections associated w/ colorectal & gynecological surgery (usually in combination w/ aminoglycosides for the aerobes). Treatment of trichomoniasis, amebiasis & gardiasis. Part of combination therapy for H. pylori. Treatment of all forms of tetanus.
Dosage/Direction for Use
Adult & childn >12 yr 500 mg every 8 hr for 7 days. Childn <12 yr 7.5 mg/kg every 8 hr. Renal insufficiency interval extended to 12 hr. Duration of therapy should not exceed 10 days.
Contraindications
1st trimester of pregnancy w/ trichomoniasis.
Special Precautions
Granulocytopenia. Pregnancy & lactation.
Adverse Reactions
GI upset, burning sensation in the tongue, metallic taste, overgrowth of Candida, rash, urticaria. Headache, dizziness, somnolence, incoordination, depression. Drug fever, darkened urine, mild leukopenia. Peripheral neuropathy, myalgia & paresthesia after high doses.
Drug Interactions
Alcohol, disulfiram, oral anticoagulant, acylureido-penicillins, spiramycin, rifampicin, clindamycin, tetracycline. Nalidixic acid. Warfarin, phenytoin, lithium, ciclosporin, fluorouracil.
