Adult 1-2 g once a day or in equally divided doses bid, depending on type & severity of infection. Total daily dose should not exceed 4 g. Surgical prophylaxis 1 g IV ½-2 hr pre-op. Gonorrhea 250 mg IM as single dose. ChildnSkin & skin structure infection 50-75 mg/kg/day, total daily dose should not exceed 2 g. Acute bacterial otitis media 50 mg/kg IM as single dose, not to exceed 1 g. Other serious infections 50-75 mg/kg/day given in divided doses 12 hrly. Max total daily dose: 2 g/day. Meningitis Initially 100 mg/kg/day (not to exceed 4 g/day). Daily dose may be administered once daily or in equally divided doses 12 hrly for 7-14 days.
Prolonged use may result in an overgrowth of nonsusceptible organism. History of GI disease especially colitis. Penicillin-sensitive patients. Pseudomembranous colitis. Patient w/ hepatic dysfunction & renal disease. Monitor prothrombin time in patients w/ impaired vit K synthesis or low vit K stores (eg, chronic hepatic disease & malnutrition). Pregnancy & lactation. Do not administer to hyperbilirubinemic neonates especially premature. Infant & childn. Elderly.
Pain, induration, tenderness & inj site reaction after IM administration. Rash; thrombocytosis, leukopenia; diarrhea, elevations of SGOT, SGPT & BUN. Headache or dizziness; moniliasis or vaginitis; diaphoresis & flushing.
