Tigecycline
Indications/Uses
Complicated skin & skin structure infections (cSSI) including those w/ MRSA; complicated intra-abdominal infections (cIAI); community-acquired pneumonia (CAP).
Dosage/Direction for Use
Adult Initially 100 mg, followed by 50 mg 12 every hr by IV infusion over approx 30-60 min every 12 hr. Duration of treatment: 5-14 days (cSSI or cIAI) or 7-14 days (CAP). Childn 12-17 yr 50 mg IV infusion over approx 30-60 min every 12 hr, 8-11 yr 1.2 mg/kg every 12 hr IV infusion to max of 50 mg over approx 30-60 min every 12 hr.
Contraindications
Special Precautions
Not for diabetic foot infections (DFI) & hospital-acquired or ventilator-associated pneumonia. Known hypersensitivity to tetracycline class antibiotics. Anaphylaxis/anaphylactoid reactions, pseudomembranous colitis. Permanent tooth discoloration. Monotherapy in patients w/ cIAI secondary to clinically apparent intestinal perforation. Hepatic dysfunction & failure. Photosensitivity, pseudotumor cerebri, pancreatitis, anti-anabolic action. Discontinue in patients suspected of developed pancreatitis. May result in overgrowth of non-susceptible organisms including fungi. Monitor liver function tests, coagulation & hematology parameters, amylase & lipase when used in childn. May affect ability to drive or operate machinery. Pregnancy & lactation. Childn <8 yr.
Adverse Reactions
Nausea, vomiting, diarrhea. Prolonged activated partial thromboplastin time (aPTT) & prothrombin time (PT), thrombocytopenia; hypoproteinemia, hypoglycemia, decreased appetite; dizziness, headache; phlebitis; pneumonia; abdominal pain, dyspepsia; increased AST & ALT, hyperbilirubinemia; pruritus, rash; headache, abnormal healing, inj site reaction, increased amylase, BUN.
Drug Interactions
Prothrombin time or other suitable anticoagulation test should be monitored if tigecycline is administered w/ warfarin. Reduced effectiveness of OCs. Increased serum trough conc of calcineurin inhibitors eg, tacrolimus or cyclosporine.
