Indications/Uses
Induction & maintenance of general anesth in adult & ped patients for inpatient & outpatient surgery.
Dosage/Direction for Use
Individualized dosage. Maintenance: 0.5-3% w/ or w/o concomitant use of nitrous oxide.
Contraindications
Hypersensitivity to sevoflurane or other halogenated agents. Known or suspected susceptibility to malignant hyperthermia.
Special Precautions
May trigger a skeletal muscle hypermetabolic state leading to high O2 demand & malignant hyperthermia. Induced malignant hyperthermia to those w/ inherited ryanodine receptor mutations. Patients w/ latent & overt neuromuscular disease, particularly Duchenne muscular dystrophy. Early & aggressive intervention in patient w/ hyperkalemia & resistant arrhythmias are recommended; evaluate for latent neuromuscular disease. Increasing the conc during maintenance produces dose-dependent decreases in BP; excessive decreases in BP or resp depression may be corrected by decreasing the inspired conc. Seizures (rarely). Concomitant use of succinylcholine. Pregnancy & lactation.
Adverse Reactions
Dose-dependent cardio-resp depression. Nausea & vomiting. Agitation (frequently in childn).
Drug Interactions
Potentiated action of non-depolarising muscle relaxants. Myocardium sensitization to the arrhythmogenic effect of exogenously administered adrenaline. Reduced MAC w/ benzodiazepines & opiates. Synergistic fall in heart rate, BP & resp rate w/ opioids (eg, alfentanil & sufentanil). Increased metabolism w/ CYP2E1 inducers (eg, INH & alcohol).
