Indications/Uses
Peptic ulcer, duodenal ulcer associated w/ H. pylori, gastric ulcer, GERD, hypersecretory conditions like Zollinger-Ellison syndrome, prophylaxis of acid aspiration, reflux esophagitis, dyspepsia.
Dosage/Direction for Use
Peptic ulcer 20 mg once daily. Severe cases 40 mg. Maintenance dose: 10-20 mg once daily. Duodenal ulcer 20 mg once daily for 4-8 wk. Duodenal ulcer associated w/ H. pylori Triple therapy: Omeprazole 20 mg + clarithromycin 500 mg + amoxicillin 1,000 mg bid for 10 days. (If an ulcer is present at the initiation of therapy: Continue taking 20 mg for additional 18 days); or omeprazole 20 mg + clarithromycin 250 mg + metronidazole 500 mg or tinidazole 500 mg each given bid for 1 wk; or 20 mg once daily + amoxicillin 500 mg + metronidazole 500 mg both given tid for 1 wk. Dual therapy: 40 mg once daily + clarithromycin 500 mg tid for 14 days. (If an ulcer is present at the initiation of therapy: Continue taking 20 mg for additional 14 days); 40 mg once daily + amoxicillin 750 mg bid for 2 wk. Gastric ulcer 20 mg once daily for 4 wk. Patient w/ poorly responsive gastric ulcer 40 mg once daily for 8 wk. GERD 20-40 mg once daily for 4-12 wk. Maintenance therapy: 20 mg once daily. Dyspepsia Initially 10 mg once daily. Patient w/ epigastric pain or discomfort w/ or w/o heartburn 20 mg once daily for 4 wk. Patient w/ hepatic impairment 10-20 mg once daily.
Administration
Should be taken with food: Take immediately before meals.
Contraindications
Special Precautions
Exclude malignancy. Hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Diarrhea, skin rashes & headache; reversible confusional states in severely ill patients.
Drug Interactions
Interferes absorption of ketoconazole, ampicillin, Fe salts, digoxin, cyanocobalamin. Cyclosporine, disulfiram, benzodiazepines. Increased t1/2 & plasma levels; decreased total clearance of diazepam. Reduced plasma clearance & increased t1/2 of phenytoin. Delayed absorption & reduced bioavailability w/ sucralfate.
