Ceftazidime pentahydrate
Indications/Uses
Nosocomial pneumonia, broncho-pulmonary infections in cystic fibrosis, bacterial meningitis, chronic suppurative otitis media, malignant otitis externa, complicated UTI, complicated skin & soft tissue infections, complicated intra-abdominal infections, bone & joint infections, peritonitis associated w/ dialysis in patients on CAPD. Treatment of bacteraemia that occurs in association w/ or is suspected to be associated w/ other infection. Management of neutropenic patients w/ fever suspected to be due to a bacterial infection. Peri-operative prophylaxis for patients undergoing trans-urethral resection of the prostate (TURP).
Dosage/Direction for Use
Intermittent IV Adult & childn ≥40 kg Broncho-pulmonary infections in cystic fibrosis 100-150 mg/kg/day every 8 hr. Max daily dose: 9 g. Febrile neutropenia, nosocomial pneumonia, bacterial meningitis, bacteraemia 2 g every 8 hr. Bone & joint infections, complicated skin & soft tissue infections, complicated intra-abdominal infections, peritonitis associated w/ dialysis in patients on CAPD, chronic suppurative otitis media, malignant otitis externa 1-2 g every 8 hr. Complicated UTI 1-2 g every 8 hr or 12 hr. Pre-op prophylaxis for TURP 1 g at induction of anaesth, & 2nd dose at catheter removal. Infant, toddler >2 mth & childn <40 kg Complicated UTI, chronic suppurative otitis media, malignant otitis externa, bone & joint infections, complicated skin & soft tissue infections, complicated intra-abdominal infections & peritonitis associated w/ dialysis in patient on CAPD 100-150 mg/kg/day in 3 divided doses. Max daily dose: 6 g. Neutropenia, broncho-pulmonary infections in cystic fibrosis, bacterial meningitis, bacteraemia 150 mg/kg/day in 3 divided doses. Max daily dose: 6 g. Neonate & infant ≤2 mth 25-60 mg/kg/day in 2 divided doses in most infections. Elderly >80 yr Not to exceed 3 g daily dose. Patient w/ renal impairment Initial loading dose of 1 g, maintenance dose should be based on CrCl. Continuous infusion Febrile neutropenia, nosocomial pneumonia, bacterial meningitis, bacteraemia, bone & joint infection, complicated skin & soft tissue infection, complicated intra-abdominal infection, peritonitis associated w/ dialysis in patient on CAPD Adult & childn ≥40 kg Loading dose: 2 g followed by 4-6 g continuous infusion every 24 hr. Infant, toddler >2 mth & childn <40 kg Loading dose: 60-100 mg/kg followed by a continuous infusion 100-200 mg/kg/day. Max daily dose: 6 g.
Contraindications
Hypersensitivity to ceftazidime or to any other cephalosporin. History of severe hypersensitivity (eg, anaphylactic reaction) to any other type of β-lactam antibacterial agent (penicillins, monobactams & carbapenems).
Special Precautions
Discontinue immediately if severe hypersensitivity reactions occurs. Do not use as a single agent unless the pathogen is known to be susceptible. Antibacterial agent-associated colitis & pseudo-membranous colitis may occur. Prolonged use may result in the overgrowth of non-susceptible organisms (eg, enterococci, fungi). Concurrent use w/ nephrotoxic medicinal products such as aminoglycosides or potent diuretics (eg, furosemide). False +ve w/ Cu reduction methods (Benedict's, Fehling's, Clinitest); may interfere w/ cross-matching of blood. Renal impairment. Pregnancy.
Adverse Reactions
Eosinophilia, thrombocytosis; phlebitis or thrombophlebitis; diarrhoea, transient increases in hepatic enzymes; maculopapular or urticarial rash; pain &/or inflammation following IM inj; +ve Coomb's test.
Drug Interactions
Concurrent use of high doses w/ nephrotoxic medicinal products may adversely affect renal function. Concomitant use w/ chloramphenicol may have antagonistic effect.
