Epoetin α (recombinant human erythropoetin)
Indications/Uses
Treatment of anemia, especially renal anemia from renal function insufficiency including hemodialysis & non-hemodialysis of chronic renal failure.
Dosage/Direction for Use
IV/SC Initial Dose: Patient receiving hemodialysis 100-150 IU/kg/wk. Patient not on hemodialysis 75-100 IU/kg/wk. If needed, increase dosage after 4 wk to 15-30 IU/kg/wk. Max: 30 IU/kg/wk. Maintenance: Start w/ 2/3 of the initial dose when hematocrit has reached 30-33% v/v &/or hemoglobin is 100-110 g/L. All doses to be taken 2-3 times wkly.
Contraindications
Uncontrollable, severe HTN. Combined infections.
Special Precautions
Check hematocrit regularly (ie, once a wk at the early stage & once every 2 wk during maintenance). Maintain hematocrit at 30 vol%. Adjust diet to avoid hyperkalemia. Patients w/ MI or pulmonary infarction. Monitor BP regularly. Adopt daily ferrotherapy if serum ferric conc is <100 mg/mL or if transfer in saturation is <20%. Childn, pregnancy & lactation.
Adverse Reactions
Headache, low fever, fatigue. Skin rash/urticaria. HTN, exacerbation of existing HTN & hypertensive encephalopathy. Increased blood viscosity, hepatic impairment, increased GOP & GPT. Nausea, vomiting, anorexia & diarrhea.
Drug Interactions
Folic & vit B12 insufficiency, ultra-high Al.
