Indications/Uses
Upper resp tract infections including ENT infections (eg, otitis media, sinusitis, tonsillitis, & pharyngitis); lower resp tract infections including pneumonia, acute bronchitis & acute exacerbations of chronic bronchitis; GUT infections including pyelonephritis & gonorrhea; skin & skin soft tissue infections including furunculosis, pyoderma & impetigo; septicemia; meningitis; bone & joint infections. Surgical prophylaxis (eg, biliary, gastroduodenal, CV, or orthopedic surgery).
Dosage/Direction for Use
IV Adult 750 mg-1.5 g every 8 hr for 5-10 days. Uncomplicated urinary infections, skin & skin structure infections, disseminated gonococcal infections, uncomplicated pneumonia 750 mg every 8 hr. Severe or complicated infections, bone & joint infections 1.5 g every 8 hr. Life-threatening infections or infections due to less susceptible organisms 1.5 g every 6 hr. Bacterial meningitis Do not exceed 3 g every 8 hr. Preventive use for clean-contaminated or potentially contaminated surgical procedure 1.5 g just before surgery (approx ½ to 1 hr before the initial incision), then 750 mg every 8 hr thereafter when the procedure is prolonged. Preventive use during open heart surgery 1.5 g at the induction of anesth & every 12 hr thereafter for a total of 6 g. Ped patient about 3 mth 50-100 mg/kg daily in equally divided doses every 6-8 hr. More severe or serious infections Max: 100 mg/kg daily Renal impairment CrCl >20 mL/min 750-1.5 g every 8 hr, 10-20 mL/min 750 mg every 12 hr, <10 mL/min 750 mg every 24 hr.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Evaluate renal status during therapy especially in seriously ill patients receiving max doses. Concurrent treatment w/ potent diuretics. Reduce total daily dose in patients w/ transient or persistent renal insufficiency. Overgrowth of nonsusceptible organism w/ prolonged use. History of GI disease particularly colitis. Nephrotoxicity w/ concomitant use of aminoglycosides. Mild to moderate hearing loss in ped patients may occur during treatment of meningitis. May be associated w/ fall in prothrombin activity including patients w/ renal or hepatic impairment, poor nutritional state, those receiving protracted course of antimicrobial therapy, & previously stabilized on anticoagulant therapy. Monitor prothrombin time in patients at risk & exogenous vit K as indicated. Increased risk of developing resistant bacteria when prescribed in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication. Pregnancy & lactation. Childn <3 mth. Newborn infants (prolonged drug t½). Elderly.
Adverse Reactions
Diarrhea.
Drug Interactions
May affect the gut flora & leading to lower estrogen reabsorption & reduce of combined estrogen/progesterone OCs. False +ve reaction for glucose in the urine w/ Cu reduction tests (Benedict's or Fehling's) soln or w/ Clinitest. False -ve w/ ferricyanide test.
