Dobutamine HCl
Indications/Uses
Increase heart contractility in acute heart failure associated w/ cardiogenic shock & MI. Cardiac surgery, +ve & expiratory pressure ventilation.
Dosage/Direction for Use
IV infusion Acute heart failure 2.5-10 mcg/kg/min. Dose range: 0.5-40 mcg/kg/min. Alternative to exercise in cardiac stress testing 5 mcg/kg/min for 8 min then increased by increments of 5 mcg/kg/min up to a max of 20 or 40 mcg/kg/min.
Contraindications
Hypersensitivity. Patients w/ idiopathic hypertrophic subaortic stenosis.
Special Precautions
Avoid abrupt w/drawal. Discontinue use if arrhythmia or marked ST-segment depression occurs. Continuously monitor ECG, BP, pulmonary wedge pressure & cardiac output. Correct hypovolemia before treatment. Cardiogenic shock complicated by severe hypotension; marked mechanical obstruction eg, severe valvular aortic stenosis; acute MI. Pregnancy. Childn.
Adverse Reactions
Increased heart rate, BP, ventricular ectopic activity, angina or chest pain & palpitation.
Drug Interactions
Increased cardiac output w/ concomitant use of nitroprusside increased peripheral resistance w/ β-adrenergic blocking agents. May cause ventricular arrhythmias w/ general anesth eg, halothane & cyclopropene.
