Memantine HCl
Indications/Uses
Moderate to severe Alzheimer's disease.
Dosage/Direction for Use
Adult (>18 yr) Max: 20 mg daily. Wk 1: 5 mg (½ tab in AM, no dose in PM). Wk 2: 10 mg (½ tab in AM, ½ tab in PM). Wk 3: 15 mg (1 tab in AM, ½ tab in PM). Wk 4 onwards: 20 mg (1 tab in AM, 1 tab in PM). Elderly >65 yr 20 mg/day. Patient w/ severe renal impairment (CrCl 40-60 mL/min/1.73 m2) 10 mg/day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Severe hepatic & renal disorder (CrCl <9 mL/min/1.73 m2).
Special Precautions
Patients w/ epilepsy. Avoid concomitant use of amantadine, ketamine, dextromethorphan, N-methyl-D-aspartate. May raise urine pH. Patients who have recent MI, under compensated cardiac insufficiency or untreated high BP. May impair ability to drive or operate machinery. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Fatigue, pain, HTN, dizziness, headache, constipation, vomiting, back pain, confusion, somnolence, hallucination, coughing, dyspnea.
Drug Interactions
May enhanced effects of L-dopa, dopaminergic agonists & anticholinergics. May reduce effects of barbiturate & neuroleptics. Can modify effects of antispasmodic agents, dantrolene or baclofen. Avoid concomitant use w/ NMDA-antagonists eg, amantadine, ketamine or dextromethorphan. Possible risk w/ phenytoin. Possible increased in plasma levels w/ cimetidine, ranitidine, procainamide, quinidine, quinine & nicotine. Possible of reduced serum level of hydrochlorothiazide.
