Indications/Uses
Infections due to susceptible microorganisms including resp tract, GUT, skin & skin structure, biliary tract, bone & joint; endocarditis. Peri-op prophylaxis.
Dosage/Direction for Use
Adult Mild to moderately severe infections 1.5-2 g bid or tid in equally divided doses. Severe bacterial infection 3-4 g bid or tid in equally divided doses. Severe, life-threatening infections (eg, endocarditis, septicemia) Up to 6 g daily. Reduced renal function, CrCl ≥55 mL/min Usual dose, 35-54 mL/min Usual dose at least 8 hr interval, 11-34 mL/min ½ the usual dose every 12 hr, ≤10 mL/min ½ the usual dose every 18-24 hr. Childn 12 yr (40 kg) 1.125-4.5 g daily, 7 yr (23 kg) 750 mg-3 g daily, 1 yr (10 kg) 375 mg-1.5 g daily, 4 mth (6.5 kg) 300 mg-1.2 g daily. Severe renal impairment 10% of the normal daily dose every 24 hr. Moderate impairment 25% of the normal daily dose in equally divided doses every 12 hr. Mild to moderate renal impairment 60% of the normal daily dose in equally divided dose every 12 hr.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Cross-allergenicity in penicillin-sensitive patients. False +ve reaction for glucose in urine w/ Benedict's & Fehling's soln or Clinitest tab. +ve Coombs' test. Renal impairment. Pregnancy & lactation. Premature & infant <1 mth.
Adverse Reactions
Allergic reactions including skin rashes, urticaria & anaphylaxis.
Drug Interactions
Possibly enhance the effects of warfarin. Disulfiram-like reaction w/ alcohol. Delayed renal excretion w/ probenecid.
