Amlodipine is a dihydropyridine calcium-channel blocker with general properties similar to those of nifedipine but a more prolonged duration of action.
Pharmacokinetics: Amlodipine is well absorbed following oral administration with peak blood concentrations occurring after 6-12 hrs. It has a prolonged terminal elimination t½ of 35-50 hrs and steady-state plasma concentrations are not achieved until after 7-8 days of administration. Amlodipine is extensively metabolized; metabolites are mostly excreted in urine together with <10% of a dose as unchanged drug. Amlodipine is reported to be about 97.5% bound to plasma protein.
Management of hypertension and angina pectoris.
Bezam is given by mouth, alone or in combination with other agents. In the management of hypertension, in doses equivalent to amlodipine 5 mg daily once daily. The dose may be increased, if necessary, to the equivalent of 10 mg daily. Similar doses are given in the treatment of stable angina pectoris.
Patients with known sensitivity to dihydropyridine.
Use in Renal Failure: Amlodipine is extensively metabolized to inactive metabolites with 10% excreted unchanged in the urine. It may be used in patients at normal doses. It is not dialyzable. Half-life is prolonged in patients with impaired liver function. It should therefore be administered with caution in these patients.
Use in pregnancy & lactation: Safety for human pregnancy and lactation has not been established. Accordingly, use in pregnancy is only recommended when there is no safer alternative and when the disease itself carries greater risk for the mother and child.

The most prominent side effects of calcium antagonist are related to the vasodilator effects of the drug and include peripheral edema, flushing, palpitation, hypotension and syncope. Other common adverse reactions include dizziness, headaches and lightheadedness.
Amlodipine besylate can be safely administered with thiazide diuretics, β-blocker, ACE inhibitors, NSAIDs, antibiotics and oral hypoglycemic drugs.
Store at temperatures not exceeding 30°C.
