Indications/Uses
Chronic management of hyperuricemia in adults w/ gout in conditions where urate deposition has already occurred (including a history or presence of tophus &/or gouty arthritis).
Dosage/Direction for Use
Adult 20-40 mg once daily. Higher dose is recommended if serum uric acid <6 mg/dL is not achieved after 2 wk.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Pregnancy & lactation.
Special Precautions
Serious allergic/hypersensitivity reactions, including life-threatening SJS, TEN & acute anaphylactic reaction/shock may occur. Pre-existing major CV diseases (eg, MI, stroke or unstable angina); previous hypersensitivity to allopurinol; patients w/ alteration of thyroid function. Treatment should not be started until an acute attack of gout has completely subsided. Discontinue use if gout flare occurs during treatment. Not recommended in patients w/ xanthine deposition (eg, malignant disease or Lesch-Nyhan syndrome); organ transplant recipient; rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption. Perform LFT prior to initiation of therapy & periodically thereafter. Concomitant use w/ mercaptopurine/azathioprine. May affect the ability to drive & use machine. Severe renal & hepatic impairment. Childn <18 yr.
Adverse Reactions
Gout flares; headache; diarrhea, nausea; liver function abnormalities; rash (including various types of rash reported w/ lower frequencies); edema.
Drug Interactions
Increased plasma conc w/ mercaptopurine/azathioprine. Concomitant use w/ naproxen may increase exposure. Increased metabolism & decreased efficacy w/ potent inducers of UGT enzymes. Delayed absorption w/ antacids containing Mg hydroxide & Al hydroxide.
