Desmopressin acetate
Indications/Uses
Central diabetes insipidus. Primary nocturnal enuresis in childn ≥5 yr. Symptomatic treatment of nocturia in adults associated w/ nocturnal polyuria.
Dosage/Direction for Use
SL Central diabetes insipidus Adult & childn Initially 60 mcg tds. Daily dose: 120-720 mcg. Maintenance dose: 60-120 mcg tds. Primary nocturnal enuresis Initially 120 mcg at bedtime, may be increased up to 240 mcg if ineffective. Nocturia Initially 60 mcg at bedtime, may be increased up to 120 mcg then subsequently 240 mcg wkly if ineffective.
Administration
May be taken with or without food: Place under the tongue & allow to dissolve w/o water.
Contraindications
Hypersensitivity. Habitual or psychogenic polydipsia (urine production >40 mL/kg/24 hr); known or suspected cardiac insufficiency & other conditions requiring treatment w/ diuretics; SIADH; known hyponatraemia. Moderate & severe renal insufficiency (CrCl <50 mL/min).
Special Precautions
Not to be initiated in patients suffering from other conditions increasing fluid or electrolyte imbalance. Interrupt treatment in acute illness characterised by fluid &/or electrolye imbalance eg, systemic infections, fever, gastroenteritis. Urgency/urge incontinence, organic causes for increased micturition frequency or nocturia (eg, benign prostatic hyperplasia, UTI, bladder stone/tumor), polydipsia, poorly adjusted DM; increased intracranial pressure. Risk of hyponatraemia in patients w/ low serum Na levels & high 24-hr urine vol. Limit fluid intake to least possible during period of 1 hr before until at least 8 hr after administration for primary nocturnal enuresis & nocturia. Monitor fluid restriction & serum Na in concomitant use w/ drugs inducing SIADH eg, TCAs, SSRIs, chlorpromazine, carbamazepine & NSAIDs. Pregnancy & lactation. Not to be used in elderly >65 yr.
Adverse Reactions
Headache. Adults: Hyponatraemia; dizziness; HTN; nausea, abdominal pain, diarrhoea, constipation, vomiting; bladder & urethral symptoms; oedema, fatigue.
Drug Interactions
Additive antidiuretic effect w/ TCAs, SSRIs, chlorpromazine, carbamazepine, sulfonylurea antidiabetics eg, chlorpropamide. May induce water retention/hyponatraemia by NSAIDs. Increased plasma conc w/ loperamide. Decreased absorption w/ dimethicone. Decreased antidiuretic effect w/ concomitant food intake.
