Meropenem trihydrate
Indications/Uses
Pneumonias & nosocomial pneumonias, UTI, intra-abdominal infections, gynaecological infections eg, endometritis, pelvic inflammatory disease; bacterial meningitis, septicaemia, empiric treatment for presumed infections in patients w/ febrile neutropenia used as monotherapy or in combination w/ antiviral or antifungal agents.
Dosage/Direction for Use
Adult Pneumonia, UTI, gynaecological infections eg, endometritis 500 mg IV 8 hrly. Hospital acquired pneumonias, peritonitis, presumed infections in febrile neutropenic patient, septicaemia 1 g IV 8 hrly. Meningitis 2 g 8 hrly. Adult & adolescent Nosocomial infections due to Pseudomonas aeruginosa Dose up to 2 g tds. Childn >3 mth-12 yr 10-20 mg/kg 8 hrly depending on type & severity of infection, susceptibility of pathogen & patient's condition. Childn >50 kg Adult dosage. Meningitis 40 mg/kg 8 hrly. Febrile episodes in neutropenic patient 20 mg/kg 8 hrly. Nosocomial infections due to Pseudomonas aeruginosa Dose up to 40 mg/kg tds.
Contraindications
Special Precautions
History of hypersensitivity to carbapenems & β-lactam antibiotics, penicillins & cephalosporins. Overgrowth of non-susceptible organisms. Not recommended for MRSA infections. Pseudomembranous colitis. Co-administration w/ potentially nephrotoxic drugs & concomitant use of valproic acid/Na valproate. Monitor transaminase & bilirubin levels. May affect ability to drive & use machines. Renal impairment. Hepatic disease. Pregnancy & lactation. Not recommended in infant <3 mth.
Adverse Reactions
Thrombocythaemia; headache; diarrhoea, vomiting, nausea; increased ALT, AST, blood alkaline phosphatase, blood lactate dehydrogenase & γ-glutamyltransferase; rash, pruritis; inflammation, pain.
Drug Interactions
Increased elimination t½ & plasma conc w/ probenecid. Reduced serum valproic acid levels.
