Indications/Uses
HRT in postmenopausal women w/ at least 6 mth since last menstruation or surgical menopause, w/ or w/o uterus.
Dosage/Direction for Use
Initially 1 spray once daily to dry & healthy skin of inner forearm, may be increased to 2 sprays depending on clinical response. Max daily dose: 3 sprays.
Contraindications
Hypersensitivity. Known, past or suspected breast & endometrial cancer; undiagnosed genital bleeding; untreated endometrial hyperplasia. Previous or current VTE; known thrombophilic disorders; active or recent arterial thromboembolic disease. Porphyria. History of or acute liver disease.
Special Precautions
Discontinue use if VTE develops; jaundice or liver function deterioration, significant increased BP, new onset of migraine-type headache occur; if visual exam reveals papilledema or retinal vascular lesions. Increased risk of endometrial hyperplasia & carcinoma; breast & ovarian cancer; ischaemic stroke. Leiomyoma/uterine fibroids or endometriosis; risk factors for estrogen-dependent tumours eg, 1st-degree hereditary breast cancer; risk factors for VTE eg, use of oestrogens, older age, major surgery, prolonged immobilisation, obesity (BMI >30 kg/m2), SLE, cancer; HTN, liver disorders (eg, liver adenoma), DM w/ or w/o vascular involvement, cholelithiasis, migraine or severe headache, SLE, epilepsy, asthma, otosclerosis; CAD, cardiac dysfunction; preexisting hypertriglyceridemia. Avoid using sunscreen w/in 1 hr after treatment & use w/ caution in extreme temp conditions eg, sauna or sunbathing. May cause burning sensation on damaged skin due to ethanol. Temporarily withhold treatment for 4-6 wk if prolonged immobilisation is to follow elective surgery & not to be restarted until completely mobilised. Perform physical & medical exam eg, pelvic & breast exam prior to therapy. Concomitant use w/ anticoagulants. Renal dysfunction. Not indicated during pregnancy & lactation. Breast budding & masses in prepubertal females & males, precocious puberty & gynaecomastia in prepubertal males. Women >65 yr.
Adverse Reactions
Headache; abdominal pain, nausea; rash, pruritus; breast pain & tenderness, uterine/vag bleeding, metrorrhagia; increased/decreased wt.
Drug Interactions
Increased metabolism w/ CYP450 inducers eg, anticonvulsants (eg, phenobarb, phenytoin, carbamazepine), anti-infectives (eg, rifampicin, rifabutin; nevirapine, efavirenz, ritonavir, nelfinavir). Induced metabolism w/ St. John's wort-containing herbal prep.
