Indications/Uses
Prevention of atherothrombotic events in MI (few days until <35 days), ischaemic stroke (7 days until <6 mth) or established peripheral arterial disease; in acute coronary syndrome: Non-ST segment elevation (unstable angina or non-Q-wave MI) including patients undergoing stent placement following percutaneous coronary intervention; ST segment elevation acute MI in medically treated patients eligible for thrombolytic therapy.
Dosage/Direction for Use
Adult & elderly 75 mg as single dose. Non-ST segment elevation acute coronary syndrome (unstable angina or non-Q-wave MI) Initially 300 mg single loading dose & continue at 75 mg once daily (w/ ASA 75-325 mg daily). ST segment elevation acute MI 75 mg single daily dose, initiated w/ 300 mg loading dose in combination w/ ASA w/ or w/o thrombolytics. Elderly >75 yr Initiated w/o loading dose. Initiate combination therapy immediately after symptoms start & continue for at least 4 wk.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Presence of active pathological bleeding eg, peptic ulcer, intracranial hemorrhage. Severe hepatic impairment.
Special Precautions
Prolonged bleeding time; increased risk of bleeding from trauma, surgery, ulcers or other pathology particularly GI or IM conditions. Patients w/ genetically reduced CYP2C19 function. Monitor blood cell counts if hematologic or bleeding disorders are suspected. Discontinue use 5-7 days prior to elective surgery or CABG. Concomitant use w/ aspirin, other NSAIDs. Bleeding diatheses in patients w/ severe hepatic disease. Severe hepatic disease, severe renal impairment (CrCl 5-15 mL/min). Pregnancy & lactation.
Adverse Reactions
Haematoma, epistaxis, GI hemorrhage, diarrhoea, abdominal pain, dyspepsia, bruising, puncture site bleeding.
Drug Interactions
Increased risk of bleeding w/ aspirin, heparin, warfarin. Possible GI bleeding w/ naproxen, other NSAIDs. Reduced levels & clinical efficacy w/ CYP2C19 inhibitors eg, PPIs.
