Indications/Uses
PO Upper resp tract (alternative to 1st line therapy of acute pharyngitis/tonsilitis due to S. pyogenes occurring in individuals who cannot use 1st line therapy) & lower resp tract (acute bacterial exacerbations of COPD due to H. influenzae, M. catarrhalis or S. pneumoniae. CAP of mild severity due to S. pneumoniae or H. influenzae in patients appropriate for outpatient oral therapy), skin & skin structure infections, non-gonococcal urethritis & cervicitis due to Chlamydia trachomatis. IV Treatment of CAP caused by susceptible organism, including Legionella pneumophila. Treatment of PID caused by susceptible organism in combination w/ metronidazole.
Dosage/Direction for Use
PO Adult (including elderly) STD 1 g as a single oral dose. All other indications 500 mg once daily for 3 days. Childn 10 mg/kg daily as a single daily dose for 3 days. Susp 36-45 kg 400 mg (10 mL), 26-35 kg 300 mg (7.5 mL), 15-25 kg 200 mg (5 mL). All doses to be taken once daily for 3 days. >45 kg & elderly Dose as per adult. IV Adult w/ CAP 500 mg single dose for at least 2 days. Followed by oral azithromycin 500 mg single daily dose to complete a 7-10 day course of therapy. Adult w/ PID 500 mg single dose  for 1-2 days. Followed by oral azithromycin 250 mg single daily dose to complete a 7-day course of therapy.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort. Swallow whole.
Contraindications
Hypersensitivity to azithromycin, erythromycin or other macrolide or ketolide antibiotics.
Special Precautions
Discontinue use if allergic reaction, hepatitis occur. Overgrowth of C. difficile. Patients w/ congenital or documented QT prolongation, electrolyte disturbance, clinically relevant bradycardia, cardiac arrhythmia or insuficiency. Severe renal & hepatic impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Nausea, abdominal discomfort, vomiting, flatulence, diarrhea & loose stools. Hearing impairment, interstitial nephritis, acute renal failure, abnormal liver function, dizziness/vertigo, convulsions, headache, somnolence, QT prolongation. Deafness, tinnitus.
Drug Interactions
Theophylline, warfarin, carbamazepine, ergot alkaloids, cyclosporin, antacids, cimetidine, methylprednisolone, zidovudine, dideoxyinosine, atorvastatin, oral anticoagulants, efavirenz, fluconazole, indinavir, midazolam, nelfinavir, rifabutin, sildenafil, terfenadine, theophylline, triazolam & trimethoprim/sulfamethoxazole. Increased serum levels w/ digoxin.
