Valaciclovir
Indications/Uses
Herpes zoster (shingles). Herpes simplex of the skin & mucous membrane infection including initial & recurrent genital herpes.
Dosage/Direction for Use
Herpes zoster (shingles) Adult 1,000 mg 3 times daily for 7 days. CrCl 15-30 mL/min 1,000 mg twice daily, <15 mL/min 1,000 mg once daily. Herpes simplex infections Adult 500 mg twice daily. Duration of treatment: Recurrent episode: 5 days. Initial episode: May have to be extended to 10 days in more severe infection. CrCl 15-30 mL/min No dose modification. <15 mL/min 500 mg once daily.
Contraindications
Hypersensitivity to valaciclovir, acyclovir.
Special Precautions
Ensure adequate fluid intake particularly in elderly. Increased risk of developing neurological side effects in elderly & patients w/ renal impairment. Pregnancy & lactation.
Adverse Reactions
Drug Interactions
May increase aciclovir plasma conc. Cimetidine, probenecid, mycophenolate mofetil increase aciclovir AUC. Caution w/ higher dose administration w/ drugs which affect other aspects of renal physiology (eg, cyclosporin, tacrolimus).
