Cilostazol
Indications/Uses
Treatment of ischemic symptoms including ulceration, pain & coldness of the extremities in chronic arterial occlusion. Prevention of cerebral infarction recurrence (excluding cardiogenic cerebral embolism).
Dosage/Direction for Use
Administration
Should be taken on an empty stomach: Take at least 30 min before or 2 hr after meals.
Contraindications
Hypersensitivity. Haemorrhage, CHF. Pregnancy.
Special Precautions
Not to be administered in patients w/ cerebral infarction until condition has stabilized. Reduce dose or discontinue use if excessive increased pulse rate is observed. Patients w/ bleeding tendency or predisposition to bleeding; coronary artery stenosis; DM or abnormal glucose tolerance; HTN w/ consistently high BP; ventricular/atrial transposition, atrial fibrillation & flutter, ventricular tachycardia & fibrillation, multifocal ventricular ectopics, QTc interval prolongation; during menstruation; asymptomatic cerebral infarction. Closely monitor patients for any anginal symptoms eg, chest pain. Concomitant use w/ anticoagulants, antiplatelet drugs, thrombolytic drugs, prostaglandin E1 or its derivatives. High-fat diet. Long-term use. Severe hepatic impairment & renal impairment. Not to be used during pregnancy & lactation. Low birthwt & suckling infants, newborns, infants & childn. Elderly.
Adverse Reactions
Headache/dull headache, palpitation, dizziness, diarrhea, nausea/vomiting, rash; hepatic dysfunction, tachycardia, anemia, leukopenia; arrhythmias, abdominal pain.
Drug Interactions
Increased bleeding tendency w/ anticoagulants (eg, warfarin), antiplatelet drugs (eg, aspirin, ticlopidine HCl, clopidogrel sulfate), thrombolytic drugs (eg, urokinase, alteplase), prostaglandin E1 or its derivatives (eg, alprostadil, limaprost alfadex). Increased blood conc w/ CYP3A4 inhibitors (eg, macrolide antibiotics, HIV PIs, azole antimycotics, cimetidine, diltiazem HCl, grapefruit juice), CYP2C19 inhibitors (eg, omeprazole).
