Indications/Uses
Short-term treatment of serious or life-threatening infections eg, complicated UTI, septicemia, skin & skin structure infections, intra-abdominal infections including peritonitis & intra-abdominal abscess, lower resp tract infections.
Dosage/Direction for Use
IM/IV Adult 4-6 mg/kg daily as a single or in divided doses every 8 or 12 hr. Life-threatening infection Up to 7.5 mg/kg daily in divided doses every 8 hr. UTI Single dose of 150 mg daily or 3-4 mg/kg daily in divided doses every 12 hr. Duration of treatment: 7-14 days w/ 3-5 min for IV inj. Childn 6-7 mg/kg daily in divided doses every 8 hr. Infant & neonate >1 wk 7.5-9 mg/kg daily in divided doses every 8 hr. Premature infant & neonate <1 wk 6 mg/kg daily in divided doses or 3 mg/kg every 12 hr. Alternative administration Childn & infant 5.5-8 mg/kg daily in divided doses every 8 or 12 hr. Neonate <6 wk 4-6.5 mg/kg daily in divided doses every 12 hr. Hemodialysis Adult 2 mg/kg at the end of each dialysis period. Childn 2-2.5 mg/kg depending on the severity of the infection.
Contraindications
Hypersensitivity to netilmicin or other aminoglycosides.
Special Precautions
History of hypersensitivity or serious toxic reactions to other aminoglycosides. Patients receiving neuromuscular blocking agents including succinylcholine, tubocurarine or decamethonium or receiving massive blood transfusions containing citric anticoagulants. Neurotoxicity manifested by ototoxicity may occur especially in patients w/ history of renal impairment & who are treated w/ larger doses or longer than recommended. Monitor renal function & 8th cranial nerve in patients known or suspected of having impaired kidney function at the start or during therapy. Examine urine for an increase in protein excretion, wt loss & presence of cells or cast. Determine serum creatinine conc or BUN periodically. Reduced dose or discontinue treatment if ototoxicity or nephrotoxicity is proven. Avoid concomitant use w/ potentially neurotoxic or nephrotoxic drugs including cephaloridine, amphotericin B, streptomycin, kanamycin, acyclovir, gentamicin, tobramycin, amikacin, neomycin, vancomycin, bacitacin, polymixin B, colistin, promomycin, viomycin or cisplatin. Combination w/ strong diuretics eg, ethacrynic acid or furosemide may cause ototoxicity. Neuromuscular disorders eg, myasthenia gravis & infant botulism. Risk of syndromes eg, fanconi w/ aminoaciduria & metabolic acidosis. Appropriate therapy is recommended if the excessive growth of non-susceptible organisms happen. Products containing metabisulfite Na may cause allergy-type reactions including anaphylactic & life-threatening symptoms or asthmatic episodes. Pregnancy & lactation. Premature infant & neonate. Elderly.
Adverse Reactions
Nephrotoxicity; ototoxicity; local reactions including pain in the administration site, phlebitis, hypersensitivity reactions including urticaria & maculopapular rash. Increased blood sugar, alkaline phosphatase, SGOT or SGPT, bilirubin, other abnormal LFTs, increased K, reduced Hb, WBC & platelets, eosinophilia, anemia & increased prothrombin time.
