Indications/Uses
Short-term symptomatic treatment of acute exacerbations of OA, long-term symptomatic treatment of RA. Symptomatic treatment of ankylosing spondylitis.
Dosage/Direction for Use
RA & ankylosing spondylitis 15 mg daily. May be reduced to 7.5 mg daily. OA 7.5 mg daily. May be increased to 15 mg daily. History of GI disease or risk factors for CV disease Start at 7.5 mg daily. Adolescent Max: 15 mg. Renal impairment No dose reduction in mild or moderate renal impairment (CrCl >25 mL/min). Hemodialysis patient w/ severe renal impairment (CrCl <25 mL/min) Not >7.5 mg daily.
Administration
May be taken with or without food: May be taken w/ meals if GI discomfort occurs. Swallow w/ water or other liqd in conjuction w/ food.
Contraindications
Hypersensitivity. Cross sensitivity to acetylsalicylic &/or NSAID. Asthma, nasal polyps, angioedema or urticaria. Treatment of peri-op pain in the setting of CABG surgery. Active or recent GI ulceration/perforation, Crohn's disease or ulcerative colitis, severe hepatic insufficiency, non-dialysed severe renal insufficiency & uncontrolled heart failure, overt GI bleeding, recent cerebrovascular bleeding or established systemic bleeding disorders. Severe renal impairment w/o hemodialysis. Pregnancy & breastfeeding.
Special Precautions
History of upper GI disease. Patients receiving anticoagulants. Dehydration, CHF, liver cirrhosis, nephrotic syndrome, overt renal disease, patients receiving diuretic or undergone major surgical procedures which led to hypovolemia; end-stage renal failure, frail or debilitated patients, known or risk of CV disease, HTN, fluid retention or heart failure. May mask symptoms of underlying infectious disease & impair fertility. May affect ability to drive or operate machinery. Galactose intolerance, Lapp-lactase deficiency or glucose-galactose malabsorption. Elderly.
Adverse Reactions
Dyspepsia, nausea, vomiting, abdominal pain, diarrhoea, headache. Peptic ulcer, GI bleeding, flatulence, constipation, melaena, haematemesis, ulcerative stomatitis, exacerbation of colitis & Crohn's disease, infertility in female, ovulation delayed.
Drug Interactions
Other prostaglandin synthetase inhibitors, glucocorticoids, salicylates, aspirin; anticoagulants, antiplatelets, heparin, thrombolytics, SSRIs; lithium; MTX; IUD; β-blockers, ACE inhibitors, vasodilators, diuretics; AIIA; cholestyramine; cyclosporin. Pemetrexed, oral antibiotics (sulphonylurea, nateglinide).
