Indications/Uses
Type 2 DM not adequately controlled w/ exercise & diet.
Dosage/Direction for Use
Initially ½ tab (15 mg) at breakfast. Max: 4 tab (120 mg). Renal impairment 40-50 mg. Hepatic impairment >75 mg require careful medical control. Transfer: Initial dose: ½-1 tab.
Administration
Should be taken with food: Take at the beginning of the meal. The highest dose should be taken at breakfast.
Contraindications
Hypersensitivity to sulfonamides. Type 1 DM, diabetic coma & pre-coma, diabetes complicated by acidosis & ketosis, pancreatic resection, during severe infections, pre-op, acute intermittent (hepatic) porphyria. Severe hepatic impairment. Pregnancy & lactation.
Special Precautions
Dose titration phase or upon transfer from another preparation. Severe renal disease. Patients w/ G6PD deficiency. Galactose intolerance eg, galactosaemia, the Lapp lactase deficiency or glucose-galactose malabsorption.
Adverse Reactions
Hypoglycaemia.
Drug Interactions
Increased hypoglycaemic effect w/ ACE inhibitors, allopurinol, analgesics & nonsteroidal antiphlogistics, antifungals, chloramphenicol, clarithromycin, clofibrate, coumarin anticoagulants, fluoroquinolones, heparin, MAOIs, sulfinpyrazone, sulfonamides, tetracyclines & TCAs, cyclophosphamide & derivatives, insulin & other oral antidiabetics w/ & w/o intrinsic risk of hypoglycaemia. β-receptor blocking agents, other sympatholytics (including clonidine), reserpine & guanethidine may enhance the hypoglycaemic effect & mask symptoms of hypoglycaemia. Reduced hypoglycaemic effect w/ aminoglutethimide, corticosteroids, diazoxide, OCs, sympathomimetics, rifamycins, thiazide & loop diuretics, thyroid hormones, glucagon, phenothiazines, nicotinic acid, barbiturates, rifampicin & phenytoin. H2 receptor antagonists (cimetidine, ranitidine) & alcohol may reduce or enhance hypoglycaemic effects.
