Propranolol HCl
Indications/Uses
Management of HTN, angina pectoris, anxiety, tachycardia, cardiac dysrhythmias, hypertrophic obstructive cardiomyopathy & essential tremor. Adjunctive management of thyrotoxicosis & pheochromocytoma. Prophylaxis after acute MI, migraine & angina pectoris.
Dosage/Direction for Use
Adult & childn >12 yr Angina pectoris 20 mg 3-4 times daily, increase gradually to 40 mg 3-4 times daily. Max: 200-280 mg daily. Cardiac arrhythmia 10-30 mg 3-4 times daily. HTN 20 mg 3 times daily, increasing to 40 mg 3-4 times daily after 3 days. Migraine (prophylaxis) 40 mg 2-3 times daily. Hypertrophic obstructive cardiomyopathy 10-20 mg 3-4 times daily. Childn <12 yr ½ adult dose.
Administration
Should be taken on an empty stomach.
Contraindications
2nd & 3rd degree heart block, cardiogenic shock, history of bronchospasm & metabolic acidosis.
Special Precautions
Poor cardiac reserve, overt heart failure, DM; hyperthyroidism; anesth.
Adverse Reactions
GI disturbances, muscle fatigue, lassitude. Rarely bradycardia, paresthesia, thrombocytopenia, purpura, skin rash.
Drug Interactions
May enhance effect of other myocardial depressants, Ca-antagonists & hypoglycemics. Effect diminished by isoprenaline. Peripheral vasoconstrictor effects enhanced by adrenaline & noradrenaline. Effect enhanced by adrenergic neurone blockers & catecholamine depleters.
