Indications/Uses
Gastric antisecretory when oral route is not possible eg, GERD in patient w/ esophagitis &/or severe reflux symptoms; healing of gastric ulcers associated w/ NSAIDs therapy. Short-term maintenance of hemostasis & prevention of rebleeding in patients following endoscopy for acute gastric bleeding or duodenal ulcers.
Dosage/Direction for Use
20-40 mg IV or infusion once daily. Reflux esophagitis 40 mg once daily. Symptomatic treatment for GERD, gastric ulcer associated w/ NSAIDs therapy 20 mg once daily. Maintenance of hemostasis & prevention of rebleeding of gastric & duodenal ulcer 80 mg as bolus infusion over 30 min, followed by continuous IV infusion of 8 mg/hr given for 3 days (72 hr).
Contraindications
Hypersensitivity to esomeprazole or substituted benzimidazoles & any of the excipients of Esozid. Concomitant use w/ atazanavir & nelfinavir.
Special Precautions
Significant unintentional wt loss, recurrent vomiting, dysphagia, hematemesis or melena; gastric ulcer. Exclude malignancy. Risk of GI infections eg, Salmonella & Campylobacter. Co-administration w/ clopidogrel, omeprazole & atazanavir. If co-administration w/ atazanir is unavoidable, closely monitor doses of atazanavir up to 400 mg w/ 100 mg of ritonavir, esomeprazole should not exceed 20 mg. CYP2C19 metabolized drugs. Pregnancy & lactation.
Adverse Reactions
Headache; abdominal pain, constipation, diarrhea, flatulence, nausea/vomiting; administration site reactions.
Drug Interactions
Atazanavir, warfarin & other coumarine derivatives. Decreased absorption of ketoconazole & itraconazole. Increased plasma conc of drugs metabolized by CYP2C19 eg, diazepam, citalopram, imipramine, clomipramine, phenytoin. Combination of CYP2C19 & CYP3A4 inhibitors eg, voriconazole results in doubling exposure of esomeprazole.
