Per actuation Ipratropium Br 21 mcg, fenoterol HBr 50 mcg
Indications/Uses
Prevention & treatment of symptoms in chronic obstructive airway disorders w/ reversible bronchospasm eg, bronchial asthma & especially chronic bronchitis w/ or w/o emphysema. Concomitant anti-inflammatory therapy should be considered in patients w/ bronchial asthma & steroid-responsive COPD.
Dosage/Direction for Use
Adult & childn >6 yr Acute asthma episodes 2 actuations. In more severe cases, if breathing has not noticeably improved after 5 min, 2 further actuations may be taken. Intermittent & long-term treatment: 1-2 actuations, max: 8 actuations/day (ave of 1-2 actuations 3 times daily).
Contraindications
Hypersensitivity. Hypertrophic obstructive cardiomyopathy, tachyarrhythmias.
Special Precautions
Hypersensitivity. Discontinue immediately if paradoxical bronchospasm occurs. Ocular complications. Insufficiently controlled DM, recent MI, severe organic heart or vascular disorders, hyperthyroidism, phaeochromocytoma, or w/ pre-existing urinary outflow tract obstruction. Patients w/ underlying severe heart disease eg, ischaemic heart disease, arrhythmia or severe heart failure. Hypokalemia. GI motility disturbances. Dyspnea. Prolonged use. Positive result to fenoterol tests. Pregnancy & lactation.
Adverse Reactions
Cough, dry mouth, headache, tremor, pharyngitis, nausea, dizziness, dysphonia, tachycardia, palpitations, vomiting, increased systolic BP & nervousness.
Drug Interactions
Other β-adrenergics & anticholinergics, xanthine derivatives eg, theophylline may enhance bronchodilatatory effect & increase adverse reactions. Reduction in bronchodilatation may occur w/ β-blockers. Xanthine derivatives, corticosteroids & diuretics induce hypokalemia & may result in an increased susceptibility to arrhythmias in patient receiving digoxin. Enhanced action w/ MAOIs or TCAs. Halothane, trichloroethylene & enflurane increase the susceptibility on CV effects.
