Indications/Uses
Prevents rejection of the transplanted organ. May be given for an ongoing rejection of transplanted liver, kidney, heart or other organ when any previous treatment taken was unable to control the immune response after transplantation.
Dosage/Direction for Use
Individualized dosage. Initially 0.075-0.3 mg/kg daily depending on the transplanted organ. Taken bd, usually in the morning & evening.
Administration
Should be taken on an empty stomach: Take at least 1 hr before or 2-3 hr after meals.
Contraindications
Hypersensitivity to tacrolimus, sirolimus or any macrolide (eg, erythromycin, clarithromycin, josamycin).
Special Precautions
Patients w/ existing or history of liver problems; diarrhoea for >1 day; receiving any vaccinations. Perform regular blood tests to define correct dose & adjust dose from time to time. Limit exposure to sun & UV light. Avoid grapefruit & grapefruit juice while on treatment. Contains lactose. May affect ability to drive & use machines. Pregnancy. Do not breastfeed while on treatment.
Adverse Reactions
Hyperglycemia, DM, hyperkalemia; sleeping difficulty; trembling, headache; HTN; diarrhoea, nausea; kidney problems. Pancytopenia, leukocytosis, changes in RBC count; hypomagnesemia, hypophosphatemia, hypokalemia, hypocalcemia or hyponatremia, hypervolemia, hyperuricemia, hyperlipidemia, decreased appetite, acidosis, other changes in the blood salts (seen in blood tests); anxiety symptoms, confusion & disorientation, depression, mood changes, nightmare, hallucination, mental disorders; fits, disturbances in consciousness, paresthesia (sometimes painful) in the hands & feet, dizziness, dysgraphia, nervous system disorders; blurred vision, photophobia, eye disorders; tinnitus; myocardial ischemia, tachycardia; bleeding, partial or complete blocking of blood vessels, hypotension; dyspnea, changes in the lung tissue, pleural effusion, pharyngitis, cough, flu-like symptoms; inflammations or ulcers causing abdominal pain or diarrhoea, GI bleeding, stomatitis, ascites, vomiting, abdominal pain, indigestion, constipation, flatulence, bloating, loose stools, stomach problems; changes in liver enzymes & function, jaundice, liver tissue damage & hepatitis; itching, rash, hair loss, acne, hyperhidrosis; arthralgia, limbs or back pain, muscle cramps; renal insufficiency, oliguria, dysuria or impaired urination; asthenia, fever, fluid retention, pain & discomfort, increased alkaline phosphatase in the blood, wt gain, feeling feverish; insufficient function of the transplanted organ.
Drug Interactions
Not to be taken w/ cyclosporine. Potential interaction w/ antifungal medicines eg, ketoconazole, fluconazole, itraconazole, voriconazole, clotrimazole; antibiotics, particularly macrolides eg, erythromycin, clarithromycin, josamycin, rifampicin; HIV medicines eg, ritonavir; PPIs eg, omeprazole, lansoprazole; H2 blockers eg, cimetidine; contraceptive pill or other hormone treatments w/ ethinylestradiol or danazol; Ca channel blockers eg, nifedipine, nicardipine, diltiazem & verapamil; statins; phenytoin or phenobarb; corticosteroids eg, prednisolone & methylprednisolone; nefazodone; herbal prep containing St. John's wort (Hypericum perforatum); K supplements or certain diuretics eg, amiloride, triamterene or spironolactone; NSAIDs eg, ibuprofen; anticoagulants; oral diabetes medications. May worsen kidney or nervous system problems w/ ibuprofen, amphotericin B or antivirals eg, aciclovir.
