Indications/Uses
Duodenal, gastric & stomal ulcers, reflux esophagitis, GERD maintenance, symptomatic GERD, Zollinger-Ellison syndrome. H. pylori eradication in patients w/ PUD in combination w/ antibacterial regimen.
Dosage/Direction for Use
Adult & elderly Duodenal, gastric & stomal ulcer, reflux esophagitis 10-20 mg once daily. For patients suffering from reflux esophagitis who have not healed after 8 wk, 10 or 20 mg bd for 8 wk may be administered; 20 mg bd for patients w/ severe mucosal injury. GERD maintenance 10 or 20 mg once daily. Symptomatic GERD 10 mg once daily in patients w/o esophagitis. Subsequent symptom control can be achieved using an on-demand regimen taking 10 mg once daily when needed. Zollinger-Ellison syndrome Individualised dosage. A starting dose of 60 mg daily & doses up to 100 mg daily or 60 mg bd have been used. H. pylori eradication 20 mg bd w/ clarithromycin 500 mg bd & amoxicillin 1 g bd for 1 wk. 10 mg bd w/ clarithromycin 200 mg bd & amoxicillin 750 mg bd for 1 wk. If the above treatment fails, 10 mg bd w/ metronidazole 250 mg bd & amoxicillin 750 mg bd for 1 wk is considered.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity to rabeprazole Na or substituted benzimidazoles. Pregnancy & lactation.
Special Precautions
Exclude gastric or esophageal malignancy prior to treatment. Regular surveillance for long-term treatment. Hypomagnesemia. May lead to slightly increased risk of GI infections eg, Salmonella & Campylobacter & possibly Clostridium difficile. Increased risk of fracture in patients who received high-dose & long-term proton pump inhibitor therapy. Consider temporary w/drawal of proton pump inhibitor in high-dose methotrexate administration. Associated w/ very infrequent cases of subacute cutaneous lupus erythematosus. May impair ability to drive or operate machine. Severe hepatic dysfunction. Childn.
Adverse Reactions
Headache, diarrhoea, abdominal pain, asthenia, flatulence, rash, dry mouth; infection, insomnia, dizziness, cough, pharyngitis, rhinitis, vomiting, nausea, constipation, nonspecific pain & back pain, flu-like illness.
Drug Interactions
Decreased serum levels of ketoconazole. Increased trough levels of digoxin. Substantial reduction in atazanavir exposure w/ atazanavir/ritonavir. May elevate & prolong serum levels of methotrexate &/or its metabolite hydroxymethotrexate.
