Succinylated gelatin (modified fluid gelatin) 4% w/v
Indications/Uses
Colloidal plasma vol substitute for prophylaxis & treatment of relative or absolute hypovolaemia; prophylaxis of hypotension eg, during induction of epidural or spinal anaesth; haemodilution; extracorporal circulation; vehicle soln for compatible medicaments eg, insulin.
Dosage/Direction for Use
The 1st 20-30 mL should be infused slowly & under careful observation of the patient. Compensation of minor losses of blood or plasma, pre- & intra-operative routine prophylaxis 500-1,000 mL infused over 1-3 hr. Major losses of blood or plasma Average dose is between 1,000 & 2,000 mL. Emergency situations or shock conditions Initially 500 mL administered by forced infusion over 5-10 min. Thereafter, the infusion can be continued w/ doses corresponding to actual losses of blood or plasma. Isovolaemic haemodilution Amount of infused soln corresponds to the w/drawn plasma vol. Dose should not exceed 20 mL/kg. During extracorporal circulation Normally, between 500 & 1,000 mL.
Contraindications
Hypersensitivity to gelatin. Hypervolaemia; hyperhydration; severe cardiac insufficiency, recent event of cardiac infarction; severe disturbances of blood coagulation; severe renal insufficiency.
Special Precautions
Known allergic diseases, hypernatraemia, haemorrhagic diathesis, states of dehydration, left or right ventricular insufficiency, blood coagulation disorders, renal insufficiency, chronic liver disease. During compensation of severe blood losses, monitor haematocrit & observe dilution effect on coagulation factors. Check serum electrolyte conc especially in patients w/ existing disorders of haemostasis. Check plasma protein conc 6-12 hr after an operation. Pregnancy & lactation.
Adverse Reactions
As w/ all colloidal vol substitutes, anaphylactic or anaphylactoid reactions of varying degree of severity may occur eg, fever, urticaria or sudden flushing of face & neck.
Drug Interactions
Admixture of fat emulsions must be avoided.
