Levofloxacin

Mild, moderate or severe infections caused by susceptible organisms. Community-acquired pneumonia including multidrug resistant strains of S. pneumoniae (MDRSP); nosocomial pneumonia; chronic bronchitis (acute bacterial exacerbation); acute bacterial sinusitis; UTI (uncomplicated or complicated); acute pyelonephritis; skin or skin structure infections (uncomplicated or complicated); reduce incidence or disease progression of inhalational anthrax (postexposure).

Adult Anthrax (inhalational) 500 mg every 24 hr for 60 days as soon as possible after exposure. Chronic bronchitis (acute bacterial exacerbation) 500 mg every 24 hr for at least 7 days. Pneumonia Community-acquired: 500 mg every 24 hr for 7-14 day or 750 mg every 24 hr for 5 days (efficacy of 5-day regimen for MDRSP not established). Nosocomial: 750 mg every 24 hr for 7-14 days. Prostatitis (chronic bacteria) 500 mg every 24 hr for 28 days. Sinusitis (acute bacterial) 500 mg every 24 hr for 10-14 days or 750 mg every 24 hr for 5 days. Skin & skin structure infections Uncomplicated: 500 mg every 24 hr for 7-10 days. Complicated: 750 mg every 24 hr for 7-14 days. UTI Uncomplicated: 250 mg once daily for 3 days. Complicated, including pyelonephritis: 250 mg once daily for 10 days or 750 mg once daily for 5 days. Renal impairment Normal renal function dosing of 750 mg/day: CrCl 20-49 mL/min: 750 mg every 48 hr. CrCl 10-19 mL/min or hemodialysis/CAPD: Initially 750 mg, followed by 500 mg every 48 hr. Normal renal function dosing of 500 mg/day: CrCl 20-49 mL/min: Initially 500 mg, followed by 250 mg every 24 hr. CrCl 10-19 mL/min or hemodialysis/CAPD: Initially 500 mg, followed by 250 mg every 48 hr. Normal renal function dosing of 250 mg/day: CrCl 10-19 mL/min: 250 mg every 48 hr (except in uncomplicated UTI).

May be taken with or without food: Take at least 2 hr before or after Fe salts, antacids & sucralfate. Ensure adequate fluid intake.

Hypersensitivity to other quinolones.

Renal insufficiency, CNS disorder that may predispose to seizures or lower the seizure threshold. Avoid excessive exposure to sunlight. Monitor blood glucose in diabetics. Patient w/ prolonged QT interval, uncorrected hypokalemia. Pseudomembranous colitis may occur. Pregnancy & lactation. Childn.

Nausea, headache, diarrhea, insomnia, constipation, abdominal pain, dizziness, vomiting & other GI disorders, dyspepsia, vaginitis, rash, chest pain, pruritus, sinusitis, dyspnea, fatigue, flatulence, pain, back pain, rhinitis, anxiety, pharyngitis, CV disorders, convulsions & other CNS & peripheral disorders, earache, tinnitus, heart rate & rhythm disorders, abnormal hepatic function, arthralgia, arthritis, arthrosis, myalgia, osteomyelitis, skeletal pain, synovitis, tendonitis, tendon disorder.

Class IA or III antiarrhythmics, drugs containing multivalent cations eg, Mg, theophylline, warfarin, NSAIDs, antidiabetic agents.

Quinolones

J01MA12 - levofloxacin ; Belongs to the class of fluoroquinolones. Used in the systemic treatment of infections.
